Home Health RN Administrator in San Antonio, Texas at Choice Rehab

Lead a home health agency where your experience can shape patient care, support clinical teams, and strengthen daily operations. We are seeking an RN with at least three years of home health management experience and extensive OASIS expertise. This role is responsible for the agency’s clinical and administrative operations, regulatory compliance, staffing, and financial performance.

• Lead daily agency operations and supervise clinical and administrative staff.

• Support the delivery of timely, high-quality, patient-centered care.

• Ensure a Clinical Manager is available during operating hours and that the Administrator or Alternate Administrator is available as required.

• Maintain compliance with applicable federal, state, and local requirements, including oversight of any alternate delivery sites.

• Oversee staffing, staff education, performance evaluations, and personnel qualifications.

• Ensure patient records and required reports are accurate, complete, and submitted on time.

• Implement agency policies and procedures and respond to regulatory findings and recommendations.

• Manage budgets and resources while supporting safe, effective patient care.

• Communicate with the Governing Body, interdisciplinary team, agency staff, and community partners.

• Review patient satisfaction feedback and lead improvements in service delivery.

• Current RN license

• At least three years of home health management experience

• Extensive OASIS experience

• Required state training for agency administrators

• Valid driver’s license, auto insurance, and CPR certification

• BSN preferred

• HomeCare HomeBase experience preferred
