We are seeking a compassionate, organized, and dependable Resident Care Coordinator (RCC) to help lead our care team and ensure our residents receive exceptional, person-centered care each day.
The Resident Care Coordinator plays an important leadership role within the community, working closely with the Resident Care Director, Executive Director, care team, residents, and families. This position is ideal for someone who enjoys being hands-on, supporting and developing team members, solving problems, and building meaningful relationships with seniors and their families.
Key Responsibilities
Assist with the day-to-day coordination and oversight of resident care services.
Support the Resident Care Director in ensuring residents receive appropriate, timely, and compassionate care.
Help supervise, coach, train, and support Care Partners and other resident care team members.
Assist with scheduling and staffing to maintain appropriate coverage for resident needs.
Participate in resident assessments, service plan updates, and changes in condition as appropriate.
Monitor residents for physical, emotional, behavioral, or functional changes and promptly communicate concerns to the appropriate team members.
Maintain communication with residents, families, physicians, pharmacies, home health providers, hospice providers, and other healthcare partners as needed.
Assist with medication management processes and medication-related documentation in accordance with community policies and applicable regulations.
Help ensure resident records, documentation, and care-related information are accurate and completed timely.
Respond professionally and compassionately to resident and family questions or concerns.
Assist with onboarding, orientation, and ongoing training of resident care team members.
Promote accountability, teamwork, professionalism, and a positive workplace culture within the care department.
Help ensure compliance with company policies, state regulations, licensing requirements, and resident rights.
Participate in community meetings, care conferences, and quality improvement initiatives.
Provide hands-on resident care when necessary to support the team and ensure resident needs are met.
Assist with emergency situations and participate in the community's on-call rotation, if applicable.
Perform other duties as assigned to support residents and community operations.
Qualifications
High school diploma or equivalent required.
Previous experience in assisted living, memory care, senior living, long-term care, or a related healthcare setting strongly preferred.
Previous supervisory, lead caregiver, medication technician, or care coordination experience preferred.
Current certifications or credentials required by applicable state regulations.
CPR/First Aid certification preferred or ability to obtain upon hire.
Knowledge of resident care practices, medication management, documentation, and senior living regulations preferred.
Strong leadership, communication, organization, and problem-solving skills.
Ability to manage multiple priorities in a fast-paced environment.
Comfortable addressing performance concerns and holding team members accountable while maintaining a supportive leadership approach.
Must demonstrate compassion, patience, professionalism, and respect when working with residents, families, and team members.
Must be dependable and willing to assist with staffing needs when necessary.
Ability to meet the physical requirements of the position, including standing, walking, bending, reaching, and assisting residents as appropriate.
