POSITION SUMMARY

The Home Care Coordinator / Scheduler is a key member of the Rehoboth Elderly Care and Companionship team, responsible for coordinating and scheduling all home care services for our elderly clients. This role serves as the operational hub between clients, caregivers, and management — ensuring that every client receives the right care, from the right caregiver, at the right time. The ideal candidate is organized, compassionate, and thrives in a fast-paced environment where they can make a meaningful difference in the lives of seniors and their families.

KEY RESPONSIBILITIES

Scheduling & Coordination

  • Build, manage, and maintain caregiver schedules to ensure full and timely coverage for all client visits
  • Match caregivers to clients based on skills, personality, availability, and geographic proximity
  • Promptly fill open shifts due to call-outs, emergencies, or last-minute cancellations
  • Monitor and update schedules in real time using scheduling software; communicate changes immediately to caregivers and clients
  • Coordinate transportation needs and travel routes for caregivers as required
  • Client & Family Relations

  • Serve as the primary point of contact for clients and family members regarding scheduling, care plans, and service updates
  • Conduct intake calls and in-home assessments to gather client preferences and care requirements
  • Respond to client concerns, complaints, and service change requests with professionalism and urgency
  • Follow up regularly with clients and families to assess satisfaction and address evolving needs
  • Caregiver Management Support

  • Maintain caregiver availability, credentials, certifications, and compliance records
  • Communicate schedules, care plan updates, and client-specific instructions clearly to field staff
  • Track caregiver attendance, punctuality, and performance; escalate concerns to management
  • Support onboarding of new caregivers by coordinating training schedules and initial placements
  • Administrative & Compliance

  • Maintain accurate, up-to-date records in the agency's care management and scheduling system
  • Verify and process caregiver clock-in/clock-out data; flag discrepancies for payroll processing
  • Assist with billing documentation, Medicaid/insurance authorizations, and visit verification as needed
  • Ensure adherence to NYS Department of Health regulations, HIPAA, and agency policies
  • Prepare and distribute daily, weekly, and monthly scheduling reports for management review
  • Participate in an on-call rotation to address after-hours scheduling emergencies.
