POSITION SUMMARY

The Pharmacy Coordinator is responsible for developing and implementing programs that support our organizational goals around medication management, and continuously improve the processes involved in the medication use process across the organization, with the guiding objective of delivering high quality patient care.

The Pharmacy Coordinator plans, designs, implements and evaluates the processes involved with medication selection/procurement, prescribing, preparation/dispensing, administration and monitoring.

The Pharmacy Coordinator works collaboratively with the medical staff, the professional staff, and the pharmacy staff to ensure a high performing medication use process and quality outcomes.

This position participates in the development and review of policies and procedures to optimize drug utilization in the organization. This position supports organizational goals by providing quality customer service, participating in performance improvement efforts and by demonstrating a commitment to teamwork and cooperation.

PERFORMANCE EXPECTATIONS

The Pharmacy Coordinator demonstrates the ability to perform the department specific competencies as listed on the Assessment and Evaluation Tool. By participating in educational activities, the Pharmacy Coordinator assumes responsibility for continued professional and personal growth.

WORK ENVIRONMENT

Potential exists for exposure to the hazards and risk of the hospital environment, including exposure to infectious disease, hazardous substances, and potential injury.

Requires the ability to work at a computer 60% of the time; requires some standing and walking.

The department specific Assessment and Evaluation Tool lists the essential functions of this position.

Position Specifics

Pharmacy Coordinator Clinical:

The Coordinator provides Medication Therapy Disease Management (MTDM) services. Services include but are not limited to Anticoagulation, Pain Management, Diabetes Mellitus, Congestive Heart Failure, and Chronic Obstructive Pulmonary Disease. The Coordinator assists to develop new clinical programs and projects in alignment with the organizations goals. The Coordinator will provide supportive and clinical assistance to MTDM clinic services. This Coordinator will manage specific initiatives assigned by Pharmacy Leadership based on the needs of the pharmacy department and organization.

This position is assigned the co-chair of the Ambulatory Subcommittee.

Reporting Relationship: This position reports to the Pharmacy Manager.

Pharmacy Coordinator Medication Safety:

The Coordinator is designated by the Pharmacy Department to serve as the authoritative expert in safe medication use and is assigned oversight of the medication use system. The Coordinator organizes and manages the organizations medication safety initiatives and medication management policies and procedures. This position is integral in developing, implementing, and monitoring programs that continuously and safely improve the processes for the use of medications within the organization. In addition, this position also provides guidance and support during all regulatory surveys and inspections. This Coordinator will manage specific initiatives assigned by Pharmacy Leadership based on the needs of the pharmacy department and organization

This position co-chairs the Medication Safety Subcommittee of P&T (MedSTAR). The Coordinator also serves as the Secretary of Pharmacy and Therapeutics Committee and provides support to P&T Chairmen.

Reporting Relationship: This position reports to the Pharmacy Manager.

Pharmacy Coordinator - Informatics:

The Pharmacy Informatics Coordinator is responsible for all clinical technology projects, including the strategy, vision, planning and budget. This position leads the development, implementation, maintenance and monitoring processes for all technology that impacts Pharmacy Services.Such projects include Cerner Millennium Electronic Medical Record, CPOE design, Pharmacy Inventory Management Systems, Automated Dispensing Cabinet process and implementation, Bar-coding for Point of Care, Clinical decision support for Prescribers, Pharmacists, and Nurses, and the Pharmacy Website. This position also advises the Pharmacy procurement team.

In addition to the above AtlantiCare duties, this position also provides IT support for the contracted services within the Pharmacies at Acuity Long Term Hospital (LTACH) and Bacharach Rehabilitation Hospital (BRH).

Pharmacy Coordinator - Innovation and Analytics:

The Pharmacy Coordinator Innovation and Analytics is responsible for the coordination, execution, navigation, connectivity and overall innovative tactics of Pharmacy Practice processes and programs across the Pharmacy Service Line. This position is integral and serves as the liaison for inter-professional and interdepartmental collaborations, the dissemination of information, performs predictive analytics, financial analyses and statistics, and ensures the monitoring and surveillance of clinical process and programs, in support of maintaining the high quality, performance and value of the Service Line.

This position possesses a comprehensive knowledge of pharmacy practice, clinical analytical capabilities, computer skills aptitude, in-depth knowledge of pharmacy related regulation and compliance standards, a disciplined and organized work ethic, a flexible and agile framework, and a solid understanding of the organizations and the service lines strategic goals and objectives.

The Pharmacy Coordinator Innovation and Analytics oversees the upkeep and maintenance of all pharmacy service line scorecards, dashboards, and action plans in areas such as cost containment and savings, performance improvement, high risk clinical processes, customer experience and employee engagement. This position embraces the tools and techniques that drive standardization and improve operational efficiencies. This position also performs duties as required to meet the Service Line demands as well as other duties as assigned by the Senior Director or AVP.

Reporting Relationship: This position reports to the Corporate Director of Pharmacy.

Qualifications

Minimum Qualifications/Requirements:


  • Graduate of an accredited school of pharmacy.
  • Doctor of Pharmacy degree or greater than 5 years of active pharmacy practice
  • Current license in the State of New Jersey as a Registered Pharmacist.
  • Two years or greater of clinical pharmacy experience.
  • Excellent written and verbal communication, presentation and interpersonal skills.
  • Excellent time management, organizational and team building skills required.
  • Possesses leadership skills to further development and implementing clinical programs

Preferred Qualifications


  • MTM certification or experience
  • ACPE certification in disease specific management
  • Completed Pharmacy Postgraduate Year 1 or Board of Pharmacy Specialty certification
