Position Summary
Provides comprehensive case management services to clients receiving services within Detoxification Services, including the Center for Women and Children and Men's Detoxification. Maintains an assigned caseload across both programs, assisting clients in accessing treatment, community resources, and recovery supports while promoting successful transitions to ongoing care.
Essential Duties
- Maintain an assigned caseload of clients across the Center for Women and Children and Men's Detoxification programs.
- Complete client assessments, develop individualized service and discharge plans, and coordinate services based on client needs.
- Provide ongoing case management services, including resource linkage, treatment referrals, advocacy, crisis intervention, and recovery planning.
- Assist clients in accessing substance use treatment, behavioral health services, medical care, housing resources, benefits, transportation, and other community resources.
- Complete required client documentation, case notes, assessments, and referrals in UWITS and other required systems in a timely and accurate manner.
- Utilize Motivational Interviewing, Trauma-Informed Care, Harm Reduction, and other agency-approved practices to support client engagement and recovery.
- Respond appropriately to medical, behavioral health, and other client crises utilizing agency-approved de-escalation, suicide prevention, and emergency response procedures.
- Collaborate with multidisciplinary team members and community partners to coordinate care and promote positive client outcomes.
- Participate in case staffing, shift change, supervision, required meetings, trainings, and the Detoxification Services management on-call rotation as assigned.
- Maintain professional boundaries, uphold client confidentiality, and comply with agency policies, regulatory requirements, and documentation standards.

Secondary Duties
- Facilitate client orientations and provide education regarding available treatment and community resources.
- Maintain current knowledge of community resources, recovery services, and best practices in substance use disorder treatment.
- Maintain CPR, First Aid, Naloxone, and other required certifications.
- Attend meetings and trainings at multiple agency locations as assigned.
- Perform other functions as necessary or assigned.
Qualifications

High school diploma or equivalent required and (2 years of social services or homelessness-related experience) or any combination of education, experience, and training which provides the following knowledge, skills and abilities:
- Understanding and/or training in: Trauma Informed Approach, Motivational Interviewing, Stages of Change
- Must have, or get within 90 days of hire, and then maintain throughout your tenure as a CM, Case Management Certification through the Utah Department of Health and Human Services, Office of Substance Use Mental Health. https://sumh.utah.gov/education/certification/case-management/
- Ability to utilize all the tools provided that will assist your work (i.e. phone, computer, fax, etc.)
- Ability to interact in a professional manner with a diverse workforce, clients, and the public.
- Excellent verbal and written communication skills; attention to detail.
- Remain calm and respond appropriately in crises.
- Computer literate with proficiency in Microsoft Office.
- Ability to uphold professional boundaries, confidentiality regulations, agency policies and procedures, and interact in a professional manner with a diverse workforce, clients, and the public.
- Willingness to accept supervision and direction.
- Become CPR and First Aid Certified.
- If driving is a part of the job, must be at least 21 years of age, possess a current driver’s license, have a good driving record, and be insurable on the agency liability policy.
- Must be able to pass Utah-DHS – Office of Licensing background screening and maintain eligible determination through continuous background monitoring. Must pass pre-employment drug screen.
