About the Role:

The Weekend Day Hospice LPN plays a critical role in delivering compassionate, high-quality nursing care to patients receiving hospice services during weekend hours. This position focuses on managing and monitoring patients' physical and emotional needs, ensuring comfort and dignity in end-of-life care. The LPN collaborates closely with interdisciplinary teams, including physicians, social workers, and family members, to implement individualized care plans. By providing skilled nursing interventions and patient education, the LPN helps to alleviate symptoms and improve quality of life for patients and their families. Ultimately, this role supports the mission of hospice care by fostering a supportive environment that respects patient wishes and promotes holistic well-being during challenging times.

Minimum Qualifications:

  • Current and valid Licensed Practical Nurse (LPN) license in the state of practice within the United States.
  • Minimum of one year of clinical nursing experience, preferably in hospice, palliative care, or a related healthcare setting.
  • Basic Life Support (BLS) certification from an accredited organization.
  • Strong understanding of hospice philosophy and end-of-life care principles.
  • Ability to work independently during weekend shifts with minimal supervision.

Preferred Qualifications:

  • Experience working specifically in hospice or palliative care environments.
  • Certification in Hospice and Palliative Nursing (CHPN) or similar specialty certification.
  • Training in pain and symptom management techniques.
  • Excellent communication and interpersonal skills tailored to sensitive patient and family interactions.
  • Familiarity with electronic health record (EHR) systems used in hospice care.

Responsibilities:

  • Provide direct nursing care to hospice patients, including medication administration, symptom management, and wound care.
  • Assess patient conditions regularly and report changes to the supervising RN or physician to ensure timely interventions.
  • Collaborate with interdisciplinary team members to develop, update, and implement individualized care plans.
  • Offer emotional support and education to patients and their families regarding disease progression, symptom control, and available resources.
  • Document all nursing care activities accurately and maintain compliance with regulatory and organizational standards.
  • Respond promptly to patient and family needs during weekend shifts, ensuring continuity of care and comfort.
  • Assist with coordination of hospice services and facilitate communication between patients, families, and healthcare providers.

The Weekend Day Hospice LPN utilizes clinical nursing skills daily to assess patient conditions, administer medications, and provide symptom relief, ensuring patient comfort and safety. Strong communication skills are essential for effectively educating patients and families about care plans and addressing their concerns with empathy and clarity. Critical thinking and problem-solving abilities enable the LPN to recognize changes in patient status promptly and coordinate appropriate interventions with the healthcare team. Organizational skills are applied to maintain accurate documentation and manage multiple patient needs efficiently during weekend shifts.
