Field Case Manager

This position is listed on behalf of a partner company, who manages all applications and next steps. Our partner is looking for a Field Case Manager based in Canada.

The Field Case Manager will play a key role in supporting patients, healthcare professionals, and program stakeholders throughout the treatment access journey.
You will coordinate reimbursement activities, patient enrollment, therapy logistics, and ongoing support while ensuring a high standard of service.
The role combines clinical knowledge, case management, reimbursement navigation, and relationship management in a healthcare-focused environment.
You will work closely with prescribing physicians, clinic teams, insurers, patients, program managers, and pharmaceutical partners.
As a field-based position, you will build strong relationships across an assigned territory and may occasionally travel overnight.
The role offers an opportunity to make a tangible difference by helping patients navigate complex access pathways and obtain the therapies they need.
Success will require empathy, strong communication, sound clinical judgment, organization, and the ability to work effectively across multiple stakeholders.

Accountabilities:
  • Customize patient support program elements to meet the needs of assigned prescribing physicians, clinics, and patients.
  • Review patient information and medical records to support enrollment and completion of public and private payer documentation.
  • Identify reimbursement barriers and escalate relevant issues to the Program Manager.
  • Build strong relationships with physicians and clinic staff, providing ongoing support and training on program processes and approved initiatives.
  • Welcome patients into the program, collect required information, explain reimbursement guidelines, and coordinate access to therapy.
  • Navigate public and private reimbursement processes, including special authorizations, appeals, financial assistance, and coverage coordination.
  • Coordinate pharmacy selection, medication delivery, therapy initiation, and ongoing treatment status as required by the program.
  • Maintain communication with patients and healthcare providers while directing clinical questions or situations outside program scope to the appropriate healthcare professional.
  • Report adverse and serious adverse events according to established program procedures.
  • Provide mentorship, training, and peer support to other Field Case Managers and contribute to a positive team environment.
  • Participate in physician meetings, continuing medical education events, and educational presentations alongside pharmaceutical representatives.
  • Provide ongoing field feedback to program leadership regarding service quality, training needs, physician feedback, and opportunities for improvement.
  • Maintain accurate documentation and complete required reports, expenses, mileage records, timesheets, and other program documentation within established timelines.
  • Support additional duties and responsibilities as required, including standby or on-call coverage when applicable.
Requirements
  • Bachelor’s degree in healthcare or a related discipline, or a nursing diploma.
  • Active registration as an RN or RPN/LPN with the applicable provincial College of Nurses is required.
  • Minimum of 5 years of experience in healthcare, pharmaceuticals, insurance, or a related industry.
  • At least 3 years of nursing experience in a clinical setting is considered an asset.
  • Previous case management experience is preferred.
  • Experience with reimbursement navigation, special authorization processes, appeals, and field-based reimbursement support.
  • Knowledge of public and private healthcare reimbursement structures, systems, and processes.
  • Strong analytical skills, including the ability to interpret relevant regulations and legislation.
  • Experience across multiple therapeutic areas is an asset.
  • Pharmaceutical business administration, sales, or marketing experience is an asset.
  • Strong communication and relationship-building skills, with the ability to work effectively with patients, physicians, clinic staff, peers, and cross-functional teams.
  • Fluency in both French and English is required, including the ability to communicate with unilingual English-speaking customers, patients, and colleagues.
  • Ability to travel throughout an assigned territory, including occasional overnight travel.
  • Valid driver’s license, current auto insurance, and reliable access to transportation are required.
  • Strong organizational skills, attention to detail, empathy, professionalism, and the ability to manage multiple priorities independently.
