Job Description
Under supervision, the Community Health Worker II (CHW II) provides health education, disease management education, health care and social service navigation services, and/or case management services to patients/community residents.
Provides pertinent health education and health behavior information to patient
population/priority communities with the intent of assisting community
Provides a link between the community and health care system and navigate patients/participants.
Works closely with patients (and possibly their families) to provide health education in various settings (may include settings such as home visits
Establishes positive learning expectations for patients/participants and their families by assessing their needs and explaining information in a clear and concise manner.
Participates regularly in internal and external educational opportunities to enhance his/her own skills.
Under general direction of manager and other staff, coordinates educational sessions for project staff and other CHWs.
Creates individualized plans for patients/participants including goal setting together with patients/participants.
Under supervision, assists patients/participants/community members in obtaining the appropriate needed health services (e.g., doctors, follow-up exams, medications, referrals, etc.).
Responsible for continually updating his/her understanding of health-related concepts and sharing that knowledge.
Responsible for communicating needs/ questions/ concerns with supervisor / other team members.
Provides basic case management for individuals and families as appropriate. Follows-up on all issues until resolved or the proper authorities are notified.
Works with supervisor to respond to more complex cases appropriately.
Links patients/participants /community members with available health care and social services
May assist in training new CHW Is
WHAT YOU WILL NEED:
- High school diploma or GED
- Two or more years of health education, health care, or related experience preferred.
- For some positions, two years of health education and navigation experience in a behavioral health setting preferred
KNOWLEDGE & SKILLS:
- Ability to provide health education, one-on-one and in groups.
- Ability to provide case management under supervision.
- Familiarity with and/or experience with working in Chicago’s West Side and/or South Side communities (resident of West Side or South Side communities preferred).
- Passion for and dedication to working with community members and providers.
- Ability to connect and build trust with diverse patient/participant population.
- Demonstrated commitment to racial and health equity.
- Punctual, reliable, and willing to learn.
- Working understanding of Microsoft Word, Access, Excel, and PowerPoint preferred.
- Valid driver’s license and own car preferred (required for some positions).
- Bilingual in Spanish may be required for some positions.
* Hours are flexible but will include some evening and weekend hours scheduled in advance
