CHMC –RN Case Manager – Per Diem-Day Shift – 7a-3:30p- Requisition 7790

Pay Range: $37.63-$49.86*

A RN Case Manager coordinates, negotiates, and manages the care of complex patients and families to facilitate achievement of quality and cost effective patient outcomes. This position works collaboratively with interdisciplinary staff internal and external to the organization. Actively participates in performance improvement and evaluation processes related to the management of patient care across the continuum.

Qualifications:

• Registered Nurse with valid NYS licensure and current registration.

• Two (2) years nursing experience required.

• Current BLS certification required.

• Possess strong critical thinking skills and clinical competency.

• Ability to effectively manage multiple cases.

• Self-motivated, independent, professional, creative and dependable.

• Ability to merge clinical and financial processes.

• Ability to work with others as a team to ensure quality patient care and efficiency.

• Familiarity with disease management, best practice and current treatment regimens.

• Ability to interpret Medicare, Medicaid and commercial insurance plans and benefits.

• Ability to multitask, problem solve complex situations, work around interruptions, and cope with stressful situations.

• Basic computer knowledge.

• Maintaining confidentiality of all patient information.

• Maintaining quality assurance and compliance with all regulatory agencies.

• Ability to implement and support procedures and programs pertaining to patient safety and security.

• Exhibit proficient communication skills and ability to deal efficiently and effectively with a variety of personalities.

• Ability to sustain attention to details and accuracy as it pertains to the monitoring and documentation of information.
