Blueprint for Health Licensed Mental Health Clinician at Gifford Health Care – Randolph , Vermont

Gifford Health Care is seeking a Blueprint for Health Licensed Mental Health Clinician to work in our Pregnancy Intention Initiative Practices.

The Mental Health Clinician role will support patients with unique social-emotional needs in Gifford’s Pregnancy Intention Initiative practices. This position will provide enhanced health and psychosocial screening, brief intervention, and referral to more intensive treatment or services as needed. Additionally, this role will collaborate with other Gifford departments and community partners to ensure patients are receiving resources to meet their needs. This position is part of the Blueprint for Health Pilot Expansion and is subject to Blueprint deliverables.

Strong time management/organization skills, ability to multi-task and communicate effectively in a professional manner are fundamental to the role.
