LPN Care Coordinator at Pro Care Innovations – Little Rock, Arkansas

Pro Care Innovations is looking for a LPN/LVN to join our team. We are looking for Care Team Coordinators to provide care within the CCM/PCM Service. This person will provide at least 20 minutes of care coordination for Medicare patients each month via our telehealth platform. The key tasks within your job will include telephone interactions with patients, Patient Education documentation of patient interactions, care plan development, chart reviews, care coordination between providers and other facilities, and the ability to work well with the patients and providers to optimize patient care.

This type of case management requires a person with an independent work ethic and strong communication skills. Additionally, an emphasis on person-in-environment and systems theory is helpful in providing a high level of patient-focused, compassionate care. As a member of our care team, you will have the opportunity to help improve the quality of life for our patients by delivering one-on-one telephonic case management.

This is a remote position. Equipment is provided.

Responsibilities:

• Verifying Patient eligibility of Principal Care Management (PCM)/ Chronic Care Management (CCM)
• Outbound calls to patients to educate and obtain consent to enroll in the PCM/CCM program
• Outbound calls patients to provide a monthly PCM/CCM service with support towards the patient’s Chronic Conditions. Including Care Plans.
• Documenting and submitting each Patient engagement with a PCM/CCM report within PCI internal systems and Clinical EMRs

Requirements:

• · At least 2 year of health care experience preferably with Electronic Medical Records (EMRs)
  · Must be a self-starter with excellent patient assessment skills.
  · Understanding of common Medical Terminology
  · Strong computer literacy (Microsoft Windows, keyboarding skills, strong systems aptitude, etc.)
  · Multitasking skills including strong computer and telephone skills with the ability to effectively search for and type information on the computer navigate through multiple windows and screens quickly, and inputting information accurately while keeping pace with the call.
  · Excellent verbal and written communication skills.
  · Strong interpersonal skills with patients, Primary Care Physicians, Specialists, and colleagues are a must.
  Comfortable with outbound calling and enrolling patients into Medicare program

Job Type: Full-time - 40 hours per week
Pay information and structure:
Care Coordinators are paid on commission with the benefit of a guaranteed hourly wage.
We do not limit income, the more patients you serve, the more you can earn!
Guaranteed hourly wage: $17.00 - $19.00 per hour - based on education and experience.
