Are you a passionate Speech Therapist looking for flexible PRN work in a supportive and rewarding home health setting? We are seeking a dedicated professional to join our compassionate team and help patients regain independence and quality of life through personalized therapy services.

• Perform comprehensive speech and language evaluations in patients’ homes.

• Develop and implement individualized therapy plans in collaboration with physicians and care teams.

• Deliver targeted interventions to enhance communication, swallowing, and cognitive-communication skills.

• Educate patients and caregivers on therapy strategies and techniques to support continued progress.

• Document all services and patient responses in a timely and accurate manner.

• Reassess treatment goals regularly and modify plans as needed to achieve optimal outcomes.

• Communicate and coordinate effectively with the interdisciplinary team to ensure quality care.

• Maintain compliance with HIPAA, Medicare, and company documentation standards.

• Engage in ongoing professional development and stay current on best practices.

• Graduate of an accredited Speech-Language Pathology program.

• Current and valid Speech Therapy license for Maryland.

• Minimum of 2 years of clinical experience, preferably in home health or skilled nursing.

• Proficient in assessment, treatment planning, and delivering therapeutic interventions to adult and elderly populations.

• Familiarity with OASIS documentation is preferred.

• Proficient with EMR systems (Kinnser/WellSky or Homecare Homebase preferred).

• Strong communication, time management, and problem-solving skills.

• Ability to work independently and manage a flexible PRN schedule.

• Reliable transportation and a valid driver’s license are required.
