About This Role:
The IHS Designated Coordinator is responsible for meeting with, setting up, and providing ongoing training and support to individuals accessing Accra’s Individualized Home Supports with Training (IHS) program.
**This position requires travel to clients' homes in the surrounding Metro counties**
Requirements:Must meet one of the following requirements.
- Bachelor degree in a field related to Human Services, Education, or Health and one year of full-time work experience providing Direct Care services to persons with disabilities or persons age 65 and older, or equivalent work experience providing care or education to vulnerable adults or children.
- Associate degree in a field related to Human Services, Education, or Health and two years of full-time work experience providing Direct Care services to persons with disabilities or persons age 65 and older, or equivalent work experience providing care or education to vulnerable adults or children.
- Diploma in a field related to Human Services, Education, or Health from an accredited postsecondary institution and three years of full-time work experience providing Direct Care services to persons with disabilities or persons age 65 and older or equivalent work experience providing care or education to vulnerable adults or children.
- Minimum of 50 hours of education and training related to Human Services and disabilities; and four years of full-time work experience providing Direct Care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the qualifications identified in clauses (1) to (3).
Additional requirements:
- 1 year experience in a Human Service field
- Demonstrated creativity, critical thinking, and problem-solving skills
- Excellent organizational, problem solving, communication, and interpersonal skills.
- Must pass a background study with the Minnesota Department of Human Services.
- Have a valid driver’s license, clean driving record and reliable transportation to use for work driving purposes.
- Ability to travel independently across the Accra geographic service areas.
