Position Summary We’re seeking an experienced Home Health Physical Therapist to deliver high-quality, patient-centered care in the home setting. This role offers autonomy, flexibility, and strong interdisciplinary support.

Responsibilities • Perform comprehensive patient assessments • Develop and implement individualized care plans • Provide skilled PT to improve mobility, strength, and function • Educate patients/caregivers on exercises and safety • Document visits accurately in EMR • Collaborate with physicians and care team • Ensure compliance with Medicare & HIPAA standards

Qualifications • Graduate of accredited PT program • Active Virginia PT license • Minimum 3 years clinical experience (home health preferred) • OASIS experience preferred • EMR experience (WellSky/Kinnser/HCHB a plus) • Ability to supervise PTAs • Reliable transportation & valid driver’s license
