About This Position

Reentry Caseworker

OVERVIEW:
The Reentry Caseworker provides direct, participant-centered case management and program support for individuals enrolled in IMAN's Green Reentry programs. This role promotes participant stability, engagement, and long-term success through structured case management, educational programming, and coordinated services. The Reentry Caseworker collaborates closely with participants, instructors, therapists, housing staff, and partner organizations to ensure consistent support, strong program participation, and successful reentry outcomes.

KEY RESPONSIBILITIES:
Case Management & Participant Support
  • Provide direct case management services, including weekly one-on-one check-ins with assigned participants.
  • Develop and maintain Individual Success Plans focused on reentry, education, employment, and personal development goals.
  • Monitor participant progress, engagement, morale, and overall wellbeing; identify needs and refer participants to appropriate services, including housing, mental health, healthcare, public benefits, and workforce programs.
  • Participate as a member of each participant's CARE Team alongside instructors, therapists, and support staff.
  • Activate and participate in CARE Team meetings when additional support or intervention is needed.
  • Maintain consistent communication with program staff, instructors, and service providers involved in participant care.
Group Facilitation & Program Engagement
  • Facilitate or co-facilitate group sessions, including Cognitive Behavioral Interventions (CBI), life skills, job readiness, financial literacy, and restorative practices.
  • Support positive group dynamics and maintain safe, structured learning environments.
Program Operations & Coordination
  • Assist with program operations to ensure smooth execution of daily and weekend activities.
  • Coordinate logistics including meal setup and distribution, transportation to and from program sites, and organization of materials and resources.
  • Serve as a liaison to instructors, facilities staff, contractors, and partner organizations.
  • Accompany participants and staff on scheduled off-site and overnight excursions (up to three per year, including camping).
Documentation & Reporting
  • Maintain accurate and timely case notes, attendance records, and participant documentation in designated systems.
  • Maintain accurate records of participant attendance, punctuality, engagement, and progress.
  • Submit timely case notes and required documentation in designated data systems.
  • Assist with cohort summaries and program reports.
Team Collaboration & Organizational Participation
  • Participate in staff meetings, trainings, professional development, and case consultations.
  • Work with IMAN's Communications team to capture program moments, photos, and success stories.
  • Perform other duties as assigned to support program goals and participant success.
Program Focus
This role involves supporting multiple program areas, including:
  • Housing Support
  • GRE Workforce Development Programs (off-site and on-site)
  • Weekend Warriors Program Coordination
Time allocation and focus may vary seasonally based on program schedules.
REQUIRED QUALIFICATIONS:
  • High school diploma or equivalent.
  • 2–3 years of experience in social services, human services, reentry programs, case management, housing support, or a related field.
  • Knowledge of community resources, housing supports, and social service programs.
  • Strong organizational, communication, and interpersonal skills.
  • Ability to manage multiple participants and program responsibilities simultaneously.
  • Experience facilitating group sessions or educational programming preferred.
  • Commitment to equity, inclusion, and trauma-informed practices.
  • Ability to provide weekend and night coverage, as needed.
PREFERRED QUALIFICATIONS:
  • Bachelor's degree in social work, human services, criminal justice, psychology, or related field.
  • Familiarity with data systems for case management and program reporting.
  • Experience working with justice-involved populations or housing programs.
  • Knowledge of employment, education, and social service resources in the Chicago area.
PHYSICAL DEMANDS & WORKING ENVIRONMENT:
  • Ability to sit or stand for extended periods and use a computer.
  • Ability to lift and carry up to 25–50 lbs as needed for program setup, materials, or activities.
  • Willingness to participate in off-site excursions, including overnight trips (up to three per year), and occasional field visits.
  • Work is primarily in program sites but may involve varied environments, including classrooms, housing facilities, and outdoor settings.
