Position Purpose:

• To assist in marketing the Psychiatric Department by behavioral health services. Provide psychiatric assessments and assist in the transfer of patients from the Emergency Department, Medical Units to The Psychiatric Department. Plan, organize and implement programs and services for adult psychiatric patients from the age of 18 years and older.

Essential Functions & Requirements:

• Collaborate and outreach to local Community Mental Health Centers, nursing homes and neighboring hospitals for resource sharing.
• Marketing and providing education about the SAH Psychiatry and Chemical Dependency Programs to the community.
• Provide behavioral health support to the medical units and emergency department.
• Have a solid understanding of mental illness, developmental stages, and associated treatment.
• Complete Petitions and Certifications as the Crisis Workers education and credentialing apply for Involuntary admissions.
• Conduct psychosocial assessments of patients and family members to obtain information about home environment, family relationships, perception of patient’s illness and coping resources to provide emotional support and discharge planning assistance and document same in the patient’s medical record.
• Encourages understanding and acceptance of any illness as well as the treatment plan. Gather pertinent information from medical records, hospital personnel and significant others to formulate a comprehensive discharge and post-hospital plan of care.

• Communicates patient/family issues to appropriate staff members and attending physicians. Participates in team and/or family conference to ensure patient understanding and compliance with the medical plan.
• Collaborates with patients/significant others and/or physicians on discharge planning, such as nursing home placement, halfway house, shelter or other residence placement, rehabilitation placement, home health, transportation, etc. works with the discharge planner/Psychiatric Case Worker as needed.
• Assesses and intervenes in abuse/neglect cases (all ages) presenting to the hospital and in need of inpatient services. Prepares and maintains required documentation on each case.
• Participates in the Advance Directives Program as a liaison in order to provide patients information on Advance Directives.
• Leads weekly group therapy session on inpatient unit.

Education & Certification Requirements:

• Master’s Degree – Mental Health
• 1-3 Years of Experience – Mental Health
