Job Purpose:
The Licensed Vocational/Practical Nurse responsibility is providing nursing care, under the supervision of a registered nurse, for patients of all ages in their place of residence, coordinating care with the Interdisciplinary Group/Team, patient, family, and referring agency.
Duties & Responsibilities:
- Under the direction of the RN, assists in identifying the patient’s physical, psychosocial, and environmental needs as evidenced by documentation, clinical record, team report, and evaluations.
- Participates in planning and implementing care in conjunction with the RN, in accordance with the POC.
- Applies concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease.
- Assures the continuity of care through delivery of quality patient care.
- Provides effective communication to the patient/family, team members, and other health care professionals as evidenced by clinical notes, case conferences, and communication notes and evaluations.
- Monitors assigned cases to ensure compliance with requirements of third-party payor.
- Demonstrates commitment, professional growth, and competency.
- Promotes the Hospice philosophy and administrative policies to ensure quality of care.
- Additional duties as assigned.
- Duties and responsibilities may change, and new ones may be added, at any time with or without advanced notice.
Skills and Qualifications:
- Graduate of an accredited School of Vocational/Practical Nursing.
- State license as a Licensed Vocational/Practical nurse, or a multi-state license issued by a member of the National Nurse Licensure Compact (NLC).
- Two year experience as a Licensed Vocational/Practical Nurse in a clinical care setting, Hospice experience preferred.
- Nursing skills as defined as generally accepted standards of practice.
- Good interpersonal skills.
- Current driver’s license.
- Reliable transportation and valid auto liability insurance.
Working Conditions:
- Works in patients’ place of residence in various conditions.
- Possible exposure to blood, bodily fluids, and infectious diseases.
- The ability to work a flexible schedule and extended hours.
- Ability to travel locally.
- May experience exposure to unpleasant weather conditions.
- PRN emergency calls.
Physical & Mental Requirements:
- Prolonged standing and walking required.
- Ability to lift up to 50 lbs and move patients.
- Requires ability to handle stressful situations in a calm and courteous manner at all times.
- Requires working under some stressful conditions to meet deadlines and patient needs.
- Ability to make quick decisions and resource acquisition.
- Meet patient and family individualized psychosocial needs.
- Requires eye-hand coordination and manual dexterity.
