Position: Clinical Liaison Acute Rehab & Transitional Care Units
Department: Hospital Based
Reports to: Program Director

Summary:
The Clinical Liaison acts as a coordinator between internal and external referral sources, and hospital
based rehabilitation unit operations: Acute Rehab Unit (ARU) and Transitional Care Unit (TCU). The
Clinical Liaison is responsible for establishing relationships with Case Managers, Physicians, Payors and
other referral sources. The Clinical Liaison will ensure appropriate patient placement through clinical
screenings and assessments in collaboration with Case Management, Physicians, Medical Director of
Rehab Unit and the interdisciplinary team. Performs pre-admission assessments to determine patients
candidacy for admissions and assists to facilitate admission to ARU or TCU. Achieves quantifiable
objectives set by Enduracare Acute Care management. Calls upon hospital/personnel, physicians, and
other potential referral sources to develop and manage key account within a specific geographical
territory. Responsible for following CMS guidelines for ARU & TCU admissions, ensuring a complete
Preadmission screening process, and management of a database that tracks statistical data for referrals,
admissions and follow up.
Environment: Work will be performed primarily indoors at one of our facilities, throughout all areas, including in patient rooms, and on carpeted and/or tiled floors. Work will be performed routinely around other co-workers, healthcare staff, patients, and guests. Due to the nature of facilitys business, worker may be exposed to occasional slippery floors, object on floors, chemicals, sharp objects, hazardous materials and waste (including human), blood borne pathogens, and communicable diseases, as well as high-stress medical
and/or life threatening situations.
Essential Duties and Responsibilities:
Follows approved organizational and hospital policy and procedures. Utilizes appropriate patient
information sources as well as direct communications with other members of the patients care
team to identify those patients who may need acute or transitional rehab services prior to returning
home.
Performs on-site and off-site review of patients in designated facilities in a timely and thorough
manner, so it justifies the admission decision. Pre-admission assessments must include detailed
report of hospital course, functional and medical status, projected goals as well as anticipated
rehabilitation program. Liaison must demonstrate both critical thinking skills in as well as
compliance with all regulatory requirements. Updates to the Pre-admission screening are completed
within 48 hours prior to admission to the acute rehab program. Fosters team work throughout the
entire process.
Collaborates with Case Management, Nursing, Physicians and other members of interdisciplinary
team regarding complex referrals. Communicates with tact and demonstrates effective problemsolving skills. Assures effective hand off communication to Rehabilitation care givers. Works to
ensure any equipment or special needs are communicated for patient upon admission.
Educates potential patients and families on rehabilitation and benefits of program(s). Clearly
communicates to patients, expectations and requirements for participation in program. Offers each
patient and family a tour of the rehab unit and distributes collateral materials appropriately.
Develops and maintains positive business relationships with key referrers internally, as well as
externally assigned hospital(s). Provides exceptional customer service by ensuring patient referrals
are managed and orchestrated seamlessly. Educates referral sources on rehabilitation programs and
differences in levels of care as well as specialization of hospital programs. Functions as a clinical
resource to help determine the appropriate level of rehabilitative care for patients. Advises referral
sources on rehab admission criteria. Advocates for patient access for rehabilitation.
Develops and implements strategies to grow market share in core business lines in assigned
hospital(s). Works closely with the Program Director as well as Medical Directors of ARU and TCU to
promote education programs, meet and greets, and other collaborative meetings in hospitals.
Works to achieve strategic and quantifiable goals. Represents hospital as a leader in rehabilitation
medicine in the most professional and respectful manner.
Promotes hospital rehabilitation services and programs to the external community. Is available to
assist at health fairs, educational programs, etc.
Continually strives to improve and develop the Clinical Liaison program, regularly identifying
improvements and suggest changes as needed. Supports team members in the hospital and
organization.
Perform other duties as requested.
Minimum Job Requirements:
Licensed Professional in a clinical health care specialty: RN, Physical Therapy, Occupational
Therapy, Speech Language Pathology, or Medical Social Work (MSW).
Current and valid state licensure in clinical practice area or eligible for state license transfer.
Graduate of an accredited school in clinical specialty.
Minimum two years of recent clinical care experience.
Professional appearance.
Effective verbal, written and persuasive communication skills.
Organization, time management, and ability to work with others.
At least one year experience in healthcare sales/marketing preferred.
Current State Drivers License in good standing required.
Demonstrated proficiency with computers, software and a variety of office equipment.
Navigation of electronic medical record.
Must exercise sound judgment in handling professional/confidential nature of health care sales.
Physical Requirements and Work Environment:
Moderate physical activity:
o Push, pull, move, and/or lift a minimum of ten (10) pounds to a minimum height of three (3) feet and be able to push, pull, move, and/or carry such weight a minimum distance of fifty (50) feet.
o Standing and/or walking for more than four (4) hours per day.o Bending and/or stooping for more than one (1) hour at a time.
o Sitting for more than two (2) hours at a time.
o Requires consistent computer work with repetitive typing and concentrating on computer screen.
