What You’ll Do

• Evaluate patients’ mobility, strength, balance, and functional abilities to identify therapy needs.

• Develop and implement individualized treatment plans that promote recovery and independence.

• Monitor patient progress and adjust treatment plans to support the best possible outcomes.

• Educate patients and caregivers on exercises, safe movement, and strategies to support continued progress.

• Collaborate with the healthcare team to ensure coordinated, quality patient care.

• Follow infection control practices and agency policies.

• Complete accurate clinical documentation within company guidelines.

What You’ll Bring

• Graduation from an accredited physical therapy program and successful completion of the state licensure exam.

• Current, valid Physical Therapy license.

• Current CPR certification.

• Valid driver’s license and auto insurance.

• One year of home health experience preferred.

• HomeCare HomeBase experience preferred.

Physical Requirements

• Ability to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.

• Ability to lift, carry, pull, or push up to 100 pounds.
