Position Description:

The Substance Use Health Program Manager leads the planning, implementation, and evaluation of the Mental Health and Addictions Treatment (MHAT) Hub, including 24/7 residential treatment, mobile, and drop-in services for people who use substances. This role oversees program operations, supervises the Substance Use Health Team, and ensures client centered, trauma-informed, and culturally safe care for individuals facing complex challenges related to substance use, homelessness, and mental health. As a key member of the Operational Management Team, the manager collaborates with internal and external stakeholders to maintain a high-functioning, inter-professional team and uphold service quality. If a Registered Nurse, the manager may serve as the Responsible Person in Charge (RPIC). The role also ensures compliance with provincial standards and aligns services with HUCCHC’s mission and strategic priorities to meet the needs of the diverse community it serves.

Accountability:

The Substance Use Health Program Manager reports to and is accountable to the Senior Manager, Integrated Care and Client Experience. The overall accountability is to the Executive Director.

Key Responsibilities:

Program Management:

• Oversee the MHAT Hub and substance use health services to align with HUCCHC’s mission and best practices.
• Ensure quality and accountability, meeting targets and compliance requirements.
• Manage intake, assessment, and care planning for timely and effective client services.
• Oversee electronic medical records (EMRs) to ensure accuracy, privacy, and compliance.
• Ensure program compliance with provincial legislation, accreditation, and regulatory standards.

Staff Management and Development:

• Lead recruitment, onboarding, and professional development for substance use health staff.
• Lead with a growth mindset, effectively managing change, encouraging innovation, and navigating ambiguity to adapt to evolving organizational needs.
• Provide leadership and mentorship to direct reports, ensuring alignment with organizational goal, delivering guidance and support, and conducting performance evaluations to ensure productivity and job satisfaction.
• Implement initiatives that promote staff commitment and high performance by cultivating a culture centered on collaboration, accountability, inclusivity, and innovation.
• Foster a collaborative and supportive work environment that promotes professional development, team building, and continuous improvement.
• Ensure all direct reports are informed of HUCCHC’s policies, processes, and standards, and that these are upheld consistently across the community health promotion team.
• Conduct regular performance evaluations, provide coaching, and support continuing education for team members.
• Ensure adequate staffing coverage, oversee scheduling, and secure locums/relief when necessary.
• Address and resolve staff concerns in a timely manner.
• Coordinate the supervision and evaluation of students, interns, and other learners within the team.

Program Evaluation and Quality Improvement:

• Ensure timely collection, analysis, and reporting of statistical and qualitative data for internal and external reporting requirements.
• Monitor and evaluate program outcomes to inform continuous improvements and meet reporting obligations.
• Coordinate with internal teams, partner organizations, and networks to support collaborative health planning.
• Support organizational goals by ensuring substance use health team participation in Centre-wide initiatives, events, and planning.

Community & Stakeholder Engagement:

• Build and maintain relationships with community partners, healthcare, social services, and housing providers, and organizations.
• Work collaboratively to ensure services align with equity and anti-oppression principles.
• Represent HUCCHC in community forums, inter-agency collaborations, and advocacy initiatives.
• Strengthen referral pathways and expand service offerings through partnerships.

Risk & Quality Management:

• Ensure risk management protocols, including infection control and equipment maintenance.
• Maintain comprehensive client records, conduct chart reviews, and discuss complex cases.
• Advocate for systemic improvements in mental health and addiction care.
• Develop and implement policies that align with best practices and provincial standards.
• Provide leadership in high-risk cases and crisis intervention protocols.
• Facilitate debriefing sessions and support staff after critical incidents.
• Ensure incident reports are completed and submitted within 24 hours.

Other:

• Maintain and develop professional competence through appropriate continuing education (i.e. peer interaction, literature review, conferences, courses, etc.).
• Work within the anti-racism, anti-oppression framework at Hamilton Urban Core.
• Participate in Centre-wide activities such as the annual general meeting, client-centered activities, annual events and all other special events and activities related to Hamilton Urban Core
• Participate in management and leadership meetings, providing program updates and recommendations.
• Other duties as assigned or delegated.

Qualifications:

• Minimum of a post-secondary degree in public health, community health, nursing, social work, counselling, psychology, health administration, or a related field. Master’s degree preferred.
• Registration in good standing with a relevant regulatory college (e.g., OCSWSSW, CNO, CRPO) preferred. Experience & Skills
• Minimum 5 years of progressive experience in mental health and addictions, with at least 2 years in a management role.
• Proven experience in program development, quality improvement, and team supervision.
• Strong understanding of trauma-informed care, harm reduction, and culturally safe practices.
• Experience working with diverse and marginalized populations.

• Availability for flexible hours, including occasional evenings or weekends.

Schedule:

• 8 hour shifts
• Monday - Friday 9:00 am - 5:00 pm, 12:00 pm - 8:00 pm, occasional Saturday 9:00 am - 2:00 pm
