Working Title:
Case Manager

Position Grade:
10

Position Summary:
The University of New Mexico Health Sciences Center (UNM HSC) Department of Pediatrics encompasses a statewide network of hospitals, clinics, and community-based services that are integrally linked through education and research. Guided by an abiding commitment to improving and restoring health, the department applies advanced medical science to serve New Mexico's children and support them into adulthood.

The Division of Pediatric Hematology/Oncology is seeking a full-time Social Work Case Manager to support medically complex pediatric and adult patients with cancer and bleeding disorders, as well as their families, across inpatient, outpatient, and specialty clinic settings. This position plays a critical role within the interdisciplinary care team by coordinating psychosocial resources, addressing barriers to care, and supporting continuity throughout diagnosis, treatment, survivorship, and chronic disease management.

The Social Work Case Manager is responsible for facilitating patient and family access to a broad range of local, state, and national resources, including lodging, transportation, meals, financial assistance, and other essential services that support access to care. The role requires close collaboration with social workers, counselors, nurses, providers, and administrative staff to assess needs, coordinate services, and maintain ongoing communication across inpatient and outpatient settings. Strong knowledge of New Mexico Medicaid regulations, prior authorizations, and insurance navigation is essential, including support for out-of-state referrals for specialized services such as bone marrow transplantation.

This position also supports the Ted R. Montoya Hemophilia Program and Treatment Center, a federally recognized comprehensive care program serving children and adults with bleeding disorders. Case management for this population requires coordination with Medicaid programs, specialty pharmacies, and national pharmaceutical patient-support programs to ensure continuity of treatment, access to medications, and comprehensive psychosocial support.
