Job Description

Home Health Aide (TRHH)

Reports to Clinical Manager regarding scheduling, and to the RN Case Manager for patient care and related issues. Under the general supervision of a Registered Nurse and in accordance with the medical plan of treatment and the nursing care plan, performs a variety of patient care activities. Documents patient care by reporting changes in patient's condition and charting activities performed by patient.

Performs patient care activities in accordance with established policies and procedure as outlined in care plan by:

  • Performing Post-Mortem and end of life care.
  • Giving bed or tub bath or assisting with showers, performing oral hygiene, and providing foot/skin care.
  • Assisting patient with bowel/bladder elimination through the emptying of Foley catheter drainage bags, bed pans, urinals, or commodes.
  • Applying TED hose.
  • Lifting, transferring, and turning patients with proper positioning of extremities and performing range of motion exercises as directed.
  • Reminding with medications that are ordinarily self-administered.
  • Assists in other activities of daily living by:
  • Changing the linens in both occupied and unoccupied beds.
  • Preparing meals and cleaning of items used in preparation of meals.
  • Laundering linens and patient clothing as necessary.
  • Documents patient care within EMR and within compliance of HHA plan of care.
  • Reporting changes in patient's condition and significant patient information to Nursing Supervisor.
  • Charting activities performed by the patient including personal care, ambulation and exercise, household services essential to health care at home, assistance with medications that are self-administered, changes in the patient's condition and needs, and completing appropriate records.
  • Assists with ostomy care as directed by the plan of care
  • Assists with wound care as directed by the plan of care

Performs other Functions to maintain personal competence and contribute to the overall effectiveness of the departments by:

  • Successfully complete skills validation as required.
  • Completing other job-related duties and projects as assigned.

ORGANIZATIONAL RESPONSIBILITIES

Associate complies with the following organizational requirements:

  • Attends and participates in department meetings and is accountable for all information shared.
  • Completes mandatory education, annual competencies and department specific education within established timeframes.
  • Completes annual employee health requirements within established timeframes.
  • Maintains license/certification, registration in good standing throughout fiscal year.
  • Direct patient care providers are required to maintain current BCLS (CPR) and other certifications as required by position/department.
  • Consistently utilizes appropriate universal precautions, protective equipment, and ergonomic techniques to protect patient and self.
  • Adheres to regulatory agency requirements, survey process and compliance.
  • Complies with established organization and department policies.
  • Available to work overtime in addition to working additional or other shifts and schedules when required.

Education and Experience

  • The knowledge, skills and abilities as indicated below are acquired through the successful completion of a high school diploma or equivalent is preferred. Must be a minimum of 17 years of age.

The knowledge, skills and abilities as indicated below are met in one of the following ways and will be competency-tested and verified:

  1. Must be able to successfully pass HHA skills validation and the State Board of Health 100 questions test.
  2. Must be in good standing with HHA/CNA Registry maintained by the Michigan State Department of Health and with the State of Michigan and/or individual's State of Certification.

Knowledge & Skills

  • Requires ability to read and comprehend written materials with medical terminology.
  • Demonstrates ability to communicate both orally and in writing in order to report and record patient related information accurately and appropriately.
  • Requires knowledge of and a competency in performing patient care procedures and functions and uses of equipment. Must own a watch to perform charting functions.
  • Requires ability to maintain clinical skills and knowledge by attending or completing a minimum of twelve hours of in-service annually.
  • May require special training on certain cases if needed.
  • Demonstrates evidence of physical health and physical exam upon hire and as needed. Must complete annual TB test.
  • Must have working phone and/or pager to maintain contact with agency.
  • Must have reliable transportation or access to a vehicle. Must have proof of automobile liability insurance coverage if working cases that require an Aide to provide transportation.
  • Requires ability to maintain current Certified Nurse Assistant (CNA) or Home Health Aide (HHA) and CPR certifications.

Working Conditions

  • Exposure to some unpleasant patient care activities.
  • Will be expected to submit availability upon hire and maintain that availability from that point forward unless change to availability is approved by manager.
  • Will be expected to care for any client he/she is scheduled to see.
  • Some travel via vehicle to the client's homes.

Physical Demands

  • Requires the physical ability and stamina (i.e., ability to push wheelchairs and carts, to lift or pull patients or supplies, to provide CPR, and to bend, squat, get on hands and knees, stand, sit or walk for long periods of time) to perform the essential functions of the position.
