Senior Caseworker, Patient Support & Services

This position is listed on behalf of a partner company, who manages all applications and next steps. Our partner is looking for a Senior Caseworker, Patient Support & Services based in Canada.

This role provides essential support to patients by coordinating complex cases, improving access to care, and ensuring continuity throughout their healthcare journey.
You will manage patient support activities across multiple touchpoints, helping individuals and caregivers navigate services, resources, and administrative requirements.
The position combines case management expertise, healthcare coordination, communication, and problem-solving to deliver meaningful outcomes.
You will collaborate with healthcare providers, payers, pharmacies, and internal teams to remove barriers and improve patient experiences.

Accountabilities:
  • Manage complex patient cases by coordinating services, resources, and support activities across multiple stakeholders.
  • Coordinate appointments, referrals, authorizations, documentation, and related service requirements to support timely case progression.
  • Respond to escalated patient and caregiver inquiries regarding coverage, benefits, documentation, and available support pathways.
  • Maintain accurate case records, updates, and documentation to ensure compliance, visibility, and effective case tracking.
  • Connect patients with community resources, financial assistance programs, and support services to address barriers to care and improve adherence.
  • Collaborate with healthcare providers, payers, pharmacies, and internal teams to align services and resolve gaps in patient support.
  • Identify opportunities to improve workflows, update processes, and contribute to the adoption of best practices.
  • Support consistent and efficient case management operations through strong organization and communication.
Requirements:
  • High school diploma, GED, or equivalent professional experience required.
  • Associate degree in healthcare administration, health sciences, social services, or a related field is preferred.
  • 3+ years of experience in case management, patient support services, healthcare coordination, customer operations, social services, or a related field.
  • Experience managing patient cases, coordinating services, and supporting individuals through complex processes.
  • Strong communication and interpersonal skills with the ability to interact with patients, caregivers, healthcare professionals, and internal teams.
  • Excellent organizational skills with the ability to manage multiple priorities and maintain accurate documentation.
  • Strong problem-solving abilities and attention to detail.
  • Ability to work independently while collaborating effectively in a remote environment.
  • Certification in case management, patient services, healthcare support, or customer service operations is an asset.
