Position Summary
The Licensed Practical Nurse (LPN) provides skilled nursing care to patients in the home setting under the supervision of a Registered Nurse (RN). The LPN delivers care in accordance with physician orders, established care plans, agency policies, and applicable state and federal regulations.
This role supports patient care through direct skilled services, monitoring, documentation, and communication with the interdisciplinary care team.
Reporting Structure
This position reports to the Registered Nurse (RN) or designated clinical supervisor. The LPN functions as part of the interdisciplinary care team and provides care within the established plan of care under RN supervision.
Scope of Practice
The LPN performs nursing functions within the scope of Florida LPN licensure, state and federal regulations, Medicare Conditions of Participation, and agency policies. The LPN does not independently develop care plans or perform clinical assessments outside of established RN direction.
Essential Job Functions
1. Skilled Patient Care
- Provide skilled nursing care under the supervision of an RN
- Deliver care in accordance with physician orders and established care plans
- Monitor patient condition and report changes to the RN and care team
- Support patient comfort, safety, and continuity of care
2. Clinical Tasks & Interventions
- Perform skilled nursing procedures within LPN scope of practice and agency policy
- Administer medications and treatments as permitted by state regulations and competency validation
- Provide wound care, dressing changes, catheter care, and related nursing tasks
- Assist with rehabilitative and supportive care interventions
(All tasks performed per RN direction and agency policy)
3. Patient Monitoring & Communication
- Observe and report changes in patient condition, response to treatment, or concerns
- Communicate relevant updates to supervising RN and care team
- Reinforce patient education provided by RN or care plan
4. Documentation & Compliance
- Complete timely and accurate clinical documentation in the EMR system
- Document patient care, observations, and interventions performed
- Follow Medicare requirements, state regulations, and agency policies for documentation
5. Team Collaboration
- Participate in coordination with RN and interdisciplinary care team
- Follow established care plans and notify RN of any concerns or changes
- Participate in required training, in-services, and staff meetings
Qualifications
- Graduate of an accredited Practical Nursing program required. Current Florida Licensed Practical Nurse (LPN) license in good standing required. Minimum of one (1) year of nursing experience required; home health, hospice, or acute care experience preferred.
- Current CPR certification required
- Valid driver’s license, reliable transportation, and auto insurance
- Ability to pass background screening and all compliance requirements

Work is performed in patient homes with exposure to variable environmental conditions, including pets, smoke, infectious diseases, and weather-related travel conditions.
