Job Description
Job Summary: Provide services that help restore function, improve mobility, relieve pain and prevent
permanent physical disabilities of patients suffering from injuries or disease. Restore, maintain, and
promote overall fitness and health.
The Physical Therapist contracted or employed through the Organization is responsible for providing
physical therapy services and for adherence to all conditions outlined in the Personnel Services
Agreement.
Reports to: Director of Patient Care Services
Essential Functions:
1. Providing initial and ongoing assessment to determine level of functioning, including OASIS
Assessment at appropriate time points.
2. Responsible in gathering background data, reviews referrals, selects and modifies evaluation
tools, and evaluates patients determining the level of patient’s physical and psychosocial
functioning.
3. Provides physical therapy services to patients according to a written physician's plan of care as
follows:
a. Participation in the development of the total plan of care through assessment and
evaluation of therapeutic/rehabilitative/functional status/home environment.
b. Assessment of patient’s muscle strength, mobility, gait, ROM—potential for rehab.
c. Directing physical therapy treatment and determining priority needs for physical therapy
d. Instructing patients and families/caregivers in the use and care of therapeutic appliances.
e. Reporting to physician patient’s reaction to treatment or changes in condition.
4. Responsible in developing and revising the plan of care in consultation with the physician and
other care-team members.
5. Responsible for planning, evaluation, and treatment implementation of assigned caseload.
6. Responsible in preparing clinical progress notes.
7. Responsible in supervising therapy assistants and home health aides as appropriate.
8. Responsible in evaluating outcomes.
9. Responsible in educating patients and/or families about proper safe use and recommend
methods for installation of adaptive equipment.
10. Participates in discharge planning for patient.
11. Provides in-service education programs for nursing organization personnel as needed.
12. Adhere to all Company policies and procedures.
13. Demonstrates flexible and efficient time management techniques.
14. Prioritize workload so as to meet productivity standards and applicable deadlines.
15. Establish and maintain positive interpersonal relations with internal and external customers.
16. Maintain a positive and respectful attitude and demeanor
Knowledge, Skill and Experience:
1. Minimum Education (or substitute experience) required: Currently licensed by the California
Board of Physical Therapy.
2. Minimum Experience required: Minimum of one year in the acute care setting. Two years
experience in Home Health preferred.
3. Skills Required: Must be able to maintain strict confidentiality with regard to protected and
sensitive information. Must be able to solve problems independently and confidently by applying
analytical and logical thinking. Good sense and practice of proper body mechanics.
4. Demonstrates good verbal and written communication and organization skills.
