Job Overview:
Join our dedicated team as a Community Health Worker, where you will serve as a frontline patient advocate and a vital link between health services, social services, and our local community. In this role, you will empower, support, and educate community members to help them successfully manage their health, wellness, and self-care goals. Qualifications:

Required:

  • High School Diploma or equivalent.
  • Bilingual proficiency is an essential requirement for this position.
  • Proven ability to work collaboratively, flexibly, and effectively within a multidisciplinary team.
  • Strong interpersonal skills to build positive, supportive, and trusting relationships with clients from diverse cultural and socio-economic backgrounds.
  • Excellent communication skills to interface smoothly with providers, nurse case managers, agency representatives, and office staff.

Preferred:

  • Previous healthcare background, medical field exposure, or community outreach knowledge.

Scheduling Requirements:

  • Shift: Day Shift
  • Status: Full-Time

Essential Functions:

  • Facilitate immediate client access to vital community resources, including medical care, housing, food security, transportation, employment, and mental health services.
  • Assist clients in navigating social service agencies, scheduling appointments, and completing necessary program eligibility applications.
  • Travel extensively to client homes, community locations, local agencies, and various targeted public outreach events.
  • Coach and motivate patients in the effective management of chronic health conditions, self-care practices, and understanding their prescribed care plans.
  • Help patients identify socio-economic barriers affecting their overall health, working with them to develop achievable health and social management goals.
  • Coordinate and monitor services, providing comprehensive tracking of client adherence in relation to their established care plan objectives.
  • Document all client encounters, service plans, and outcomes electronically in a timely, accurate manner; maintain comprehensive client files and submit monthly reports.
  • Act as a cultural liaison to reduce communication barriers between patients and healthcare institutions, keeping providers and clients fully aligned.
  • Continuously expand your active knowledge and understanding of local community resources, social services, and program updates.
