Job Title: Case Manager
Classification: Full-time, Non-Exempt
Reports to: Program Director / Assistant Program Director
Location: Upper West Side, New York, NY
Status & Hours: Full-time, Monday through Friday, 9:00 AM to 5:00 PM (hours may vary). Lantern staff may be asked to adjust their work schedule depending on the operational needs of the site and the agency.

About the Role: The Case Manager works with formerly homeless, mentally ill single adults in a supportive housing environment, assisting residents in stabilizing their lives, maintaining housing stability, and achieving their physical, emotional, vocational, educational, and social goals. This role requires solid clinical knowledge, experience working with individuals impacted by mental illness, substance use, legal, criminal justice, and medical issues, a passion for helping people, and the ability to work both independently and as part of an interdisciplinary team.

Work Environment: This position operates within a supportive housing site and works with a population that may experience mental illness, addiction, disability, HIV/AIDS, and other chronic health conditions.
Physical Demands: While performing the duties of this job, the employee is regularly required to stand, walk, sit, stoop, climb stairs, and/or kneel. They must also use their hands to type, handle, or feel. The employee is frequently required to talk or hear and must occasionally lift and/or move up to 10 pounds.
