Dependable Nursing is seeking an experienced contract Physical Therapist to join our growing home health team serving South San Diego County including Chula Vista, Otay Mesa, National City, and surrounding communities.

Position Details

• Contract per visit — flexible schedule

Responsibilities

• Conduct skilled physical therapy evaluations and treatment visits in the patient's home
• Complete and submit OASIS assessments accurately and timely, including Start of Care, Resumption of Care, Recertification, Transfer, and Discharge assessments.
• Develop and implement individualized plan of care in coordination with the physician and care team
• Provide gait training, strengthening, balance, transfer training, fall prevention, and ADL retraining
• Educate patients and caregivers on home exercise programs and safe functional mobility
• Maintain timely documentation in compliance with Medicare and agency standards
• Communicate clinical updates and changes in patient condition to the clinical team

• Active California Physical Therapy license in good standing
• Minimum 1 year of experience
• OASIS competency required — must be able to complete independently
• CPR certification current
• Reliable transportation and valid California driver's license
• Strong documentation and communication skills
