About This Position

Basic Function:
  • In an effort to improve health outcomes in a specific target population, works with community based organizations (CBOs) and their members to address their specific health needs.
  • This project is specific to reducing modified risk factors for cardiovascular disease within the community.
  • “Community” can include Senior Centers, Pharmacies, libraries, community-based organization partnerships, church and faith based, farmers markets, food banks, Goodwill, etc.
  • Engage community members in rural areas to reduce cardiovascular risk including such topics as benefits of physical activity, heart healthy food choices, medication adherence tools, smoking cessation resources, stress coping skills, and assuring community members have a designated Primary Care Provider.
  • May also include stroke awareness education, social determinant of health assessments & referrals, patient portal education & support, promotion of pharmacy services, medication adherence, referral to lifestyle change programs and/or Patient Activation Assessments (PAA). And other initiatives as contracts require.
  • Assists in the delivery of culturally appropriate and community-based education for the specific target population including effective strategies for promoting prevention.
  • Collaborates with community-based organizations to engage targeted community members, conduct pre-education and post-education assessment to determine impact of education provided.
  • Collaborates with other assigned team members and organizations to provide direct, logistical, and administrative support for target populations.
  • Documents outreach and engagement results with CBOs and clients in accordance with compliance and security guidelines to ensure the security and safety of client data and information.
Organizational Relationships:

Reports directly to Project Lead for Community Initiatives

Role and Responsibilities:
  • Participates in planning and improving health services by interpreting social and cultural factors of clients.
  • Will assist with community outreach events at engaged Community Based Organizations (CBO). Travels for outreach events. Flexibility in schedule and ability to work some evenings and weekends as needed.
  • Utilize effective communication and interpersonal skills, cultural competency, and motivational interviewing to engage and retain clients to help them achieve their goals, reducing the impacts of SDoH and improving health outcomes
  • Will engage targeted community members in health discussions. Will build client trust and engagement. Will deliver education to clients.
  • Leads a healthy lifestyle and serves as a positive role model within communities served.
  • Motivates members of the community to be an active engaged participant in their health
  • Takes primary responsibility for all data collection and entry required for internal project processes.
  • Provides technical assistance to others internal and external to the organization in using and evaluating the effectiveness of specific intervention approaches.
  • Other duties as assigned.

  • Aptitude to learn various aspects of healthcare and social services
  • Utilize a computer, tablet, and/or phone with appropriate and secure access to databases and web based platforms required by project. Perform accurate and complete project documentation in database and platforms daily.
  • Proficiency with computers and technology; MS Office (Outlook, Excel, Powerpoint), virtual platforms (Zoom), and mobile devices.
Minimum Education/Training Required:

High school graduate.

Minimum Experience Required:

Two years of experience in a health-related field, with preferred experience engaging priority patient populations in wellness initiatives.
