Pharmacist

The Home Infusion Pharmacists performs, coordinates, and supervises pharmacy services including the preparation and dispensing of drugs, supplies and equipment and the preparation of sterile products as they pertain to providing safe and effective therapies in the home and alternative site. This position provides clinical assessments, education, care planning and monitoring of responses to therapies and therapeutic outcomes to patients in their home or alternative sites. A pharmacy degree and current licensure (in good standing) as a Registered Pharmacist in the State of New Hampshire, or eligibility for licensure, is required. One year of home infusion pharmacy experience is preferred.
