JOB SUMMARY
-
The Health Equity Manager will be responsible to lead activities to improve the health equity for the hospital’s patients, through successful design, implementation and monitoring health equity strategy within the community that Good Shepherd Rehabilitation Network (GSRN) serves. This includes addressing social determinants of health, community engagement activities related to health as well as oversight of equity activities to mitigate actual or potential health disparities.
The role will manage the system-wide Community Health Needs Assessment and Community Service Plan, as required by regulatory agencies. The implementation of the organizational health equity goals and objectives. This position will ensure that health equity is incorporated across GSRN, including strategies to identify the health inequities that intersect with the mission of GSRN and determine evidence- and data-driven approaches to address health inequities; measure progress toward health equity; support implementation strategies across all work streams of the organization; and ensure that the organization functions in a way that is visibly and effectively promoting health equity across the continuum.

ESSENTIAL FUNCTIONS

Provide leadership and support to the Health Equity Workgroup and the DEI committee.

Identifies health care disparities in the patient populations by stratifying quality and safety data using socio-demographic characteristics of GSRN patient population.

Coordinate training, support, and resources related to health equity and assist in the dissemination of these materials to individuals, teams, and projects both within and external to the Institute.

Ensure that the health equity strategy is re-assessed annually with action plans.

Annually report outcomes to stakeholders.

Prioritize tangible, incremental, and measurable progress to promote health equity through policy and practice improvements.

Education
- Bachelor’s degree in Social work, Nursing, or other Healthcare specialty

Work Experience
- 3 to 5 years of directly related, professional experience in related role preferred.
