Job Description
Annual Non-Exempt Salary: $37,856 ($20.80 an hour)
35 Hour Work Week

Job Duties:
The Case Manager for At-Risk Youth will serve at-risk youth, with and without disabilities, and their families.
The successful candidate will be expected to:
- Work with youth who are at-risk.
- Complete intake assessments.
- Provide information and referral services.
- Conduct a youth risk assessment to determine their potential for crisis and involvement with the juvenile/criminal justice system.
- Maintain regular contact with participants and their family members to monitor progress. This includes home visits with the individuals we serve.
Knowledge, Skills & Abilities:
- Experience working with individuals with disabilities, either professionally or personally.
- Strong advocacy, networking, and organizational skills.
- Knowledge of community-based services.
- Knowledge of ADA, IDEA, 504, Social Security, ACCESS-VR, CBVH and the juvenile/criminal justice system is preferred.
Education:
- An Associates Degree is preferred; a High School Diploma or equivalent is required.
- Experience may be considered in lieu of higher education.
