This role combines clinical pharmacy expertise with program leadership, focusing on building and scaling a modern, technology-enabled pharmacy service within a fast-growing virtual care environment. You will operate as both a hands-on clinician and a strategic builder, shaping the design, scope, and delivery of pharmacy services across a national care platform. The position plays a central role in improving medication outcomes, closing care gaps, and supporting high-quality, patient-centered care. You will collaborate closely with physicians, nurses, operations, product, and technology teams to integrate pharmacy services into end-to-end care workflows. As the program expands, you will also help build and lead a growing pharmacy team. This is a high-impact opportunity to define how clinical pharmacy is delivered in next-generation virtual and in-home care models.
- Design, build, and scale the clinical pharmacy program, including service scope, care delivery model, and operational infrastructure.
- Provide direct clinical pharmacy services including medication reconciliation, therapy reviews, and identification of drug interactions, contraindications, and therapy gaps.
- Support patients with medication adherence, safety education, and health literacy during virtual and in-home care episodes.
- Collaborate with clinical leadership to define pharmacy scope-of-service opportunities and contribute to strategic program development.
- Partner with Product and Technology teams to shape pharmacy-related workflows, tools, and EMR integrations.
- Monitor and respond to prescription-related questions and clinical escalations from multidisciplinary care teams.
- Contribute to preventive care initiatives, including HEDIS measures, care gap closure, and medication-related quality programs.
- Ensure regulatory compliance across all pharmacy activities, maintaining adherence to state and federal requirements.
- Document clinical interventions accurately within EMR systems to support reporting, analytics, and care outcomes tracking.
- Support future team growth by contributing to hiring, onboarding, and development of pharmacy staff as the program scales.
- Doctor of Pharmacy (PharmD) degree from an accredited institution.
- Active pharmacist licensure, with multi-state licensure strongly preferred.
- 5+ years of clinical pharmacy experience with demonstrated ability to work independently and lead clinical initiatives.
- Proven experience building or scaling a pharmacy program, service line, or clinical pharmacy team.
- Experience in telehealth, virtual care, home health, or value-based care environments.
- Strong knowledge of medication therapy management and HEDIS pharmacy measures.
- Experience working with multidisciplinary clinical teams including physicians, NPs, RNs, and operational staff.
- Strong communication and collaboration skills with the ability to operate in fast-paced, evolving environments.
- Comfortable working in a technology-driven organization with developing structures and processes.
- Preferred: board certification (BCACP, BCPS, or equivalent) and experience with EMR platforms such as Elation.
- Preferred: prior leadership or supervisory experience in a pharmacy setting and familiarity with Medicare Advantage or Medicaid programs.
