Our MissionAs a faith-based organization, QueensCare provides, directly and in collaboration with others, accessible quality healthcare for low-income individuals and families residing in Los Angeles County.
QueensCare is a 501 (c)(3) nonprofit organization originally founded by the Franciscan Sisters of Sacred Heart, whose mission of compassion and accessible healthcare continues today. QueensCare provides screening, treatment, education, and preventive and direct care to Los Angeles area residents in need. Beyond that, we also give charitable grants to other non-profits making a difference in the City of Angels and fund scholarships for those entering medical fields with an interest in working in underserved communities.
Summary
The Associate Director (AD), Community Partnerships provides strategic, operational, and programmatic oversight of QueensCare and its affiliates’ community health services and external service delivery partnerships. This role is responsible for advancing innovative, community-responsive approaches to care and service coordination, ensuring services and partnerships are adapted to meet evolving community needs and maximize measurable impact. This AD ensures alignment with organizational goals, regulatory requirements, and high-quality standards across all service delivery and community partnerships.
The Associate Director, Community Partnerships supports implementation of QueensCare’s strategic priorities by supporting internal and external efforts to improve community health through existing and expanded services and referrals. The Associate Director supervises the Community Health Manager and supports the broader Community Health team, which primarily consists of RNs and CHWs. The Associate Director, Community Partnerships will also be primarily responsible for building and maintaining relationships with community stakeholders across Los Angeles, increasing awareness of and engagement with community health services, and supporting and expanding collaborative partnerships and referral pathways between community health services and external nonprofits and health providers.
ESSENTIAL JOB RESPONSIBILITIES
Leadership and Oversight
Provides direct supervision and leadership to the community health team.In partnership with the Chief Strategy and Program Officer, executes and oversees strategic direction for community health services and partnerships, ensuring alignment with organizational priorities and community needs.Oversees development, implementation, and evaluation of activities across Community Health services and partnerships.Ensures integration across all direct services to support whole-person, coordinated care delivery and effective referrals.Establishes team goals, performance metrics, and accountability structures; monitors outcomes and drives continuous improvement.Serves as a bridge between Community Health services and Grantmaking, strengthening external relationships and services to drive community impact.Community Partnerships
Support implementation of QueensCare’s strategic plan by assessing opportunities to expand and/or deepen services and strengthen partnerships to better serve target communities.Identifies, assesses, and establishes community partners for QHA services to support positive client and community outcomes in alignment with QueensCare’s strategic plan and organizational goals.Leads relationship building with nonprofit organizations, health providers, elected officials and other stakeholders aligned with QHA's mission to serve focus communities.Manages processes and leads outreach related to needs assessments of focus communities.Conducts regular site visits at community partner locations.Coordinates and attends community events, conferences, and other public engagements as a representative of QHA and QueensCare.Operations and Administration
Ensures efficiency, consistency, and quality across community health servicesReviews and approves budgets; monitors financial performance, including contracts, billing, and invoice reconciliation.Ensures effective staffing models, including supervision structures, productivity expectations, and workforce planning.Leads and oversees contract management and MOUs across services and external partners.Ensures effective scheduling, site coordination, and service delivery across fixed and mobile locations.Oversees coordination of infrastructure needs including equipment, mobile units, and clinical environments across internal teams.Quality, Compliance, and Risk Management
Ensures all services operate in compliance with applicable federal, state, and local regulations, including clinical documentation standards and licensing requirements.Oversees quality assurance and improvement initiatives across services.Ensures appropriate supervision, credentialing, and documentation practices for clinical and non-clinical staff.Monitors adherence to OSHA, safety protocols, and risk management practices across mobile and clinic-based services.Provides leadership during critical incidents and crisis situations, ensuring coordinated response across teams.Clinical and Service Integration
Promotes integration of community health services and new partnerships to improve patient, client, and community outcomes.Supports clinical oversight structures, including documentation review processes and adherence to care standards.Ensures appropriate referral pathways between services and external partners.Strengthens partnerships with community organizations, including faith-based organizations and schools, to expand access to care.Reporting and Communication
Prepares and presents updates, performance reports, and strategic recommendations to executive leadership and board members.Oversees preparation of comprehensive program reports, including evaluation and impact updates.Facilitates cross-program communication and collaboration to ensure alignment and shared accountability.Works with the Communications Department to create communications materials for external partnerships and community outreach.Policies and Organizational Alignment
Supports and implements the organization’s mission, vision, and values across all services and partnerships.Develops, updates, and enforces policies and procedures to ensure effective service operations.Promotes a culture of service excellence, accountability, and continuous learning.Ensures staff are trained and supported to meet organizational and regulatory expectations.Expected to uphold organizational standards of professionalism, accountability, ethics, and integrity, while fostering a respectful and high-performing work environment.Responsibilities include providing ongoing coaching and performance feedback, addressing performance or conduct concerns in a timely manner, and utilizing progressive counseling, when appropriate, to support employee accountability and development.Qualifications
Education
Bachelor’s degree in Public Health, Health Administration, Social Work, Psychology, or a related field required. Master’s degree preferred.
Experience
· Minimum of five (5) years of progressive leadership experience in community health or social services.
· At least three (3) years of direct management experience overseeing multiple programs or service lines.
· Experience supervising clinical and multidisciplinary teams.
· Experience in program development, contract management, and budget oversight.
· Experience with regulatory compliance in healthcare settings (e.g., DMH, Medi-Cal, OSHA) preferred.
· Experience working with school systems, mobile health programs, community-based services, and underserved populations preferred.
· Experience leading collaboratives of school, faith-based or community organizations preferred.
· Demonstrated experience in quality management and performance improvement.
· Experience managing crisis situations and organizational response.
Knowledge
· Knowledge of community health systems, education, and social services.
· Knowledge of interdisciplinary care models and/or collective impact models.
· Strong understanding of program evaluation and data-driven decision-making.
· Understanding of regulatory and compliance requirements in clinical and community health settings preferred.
· Familiarity with EHR systems and healthcare operations preferred.

The pay range for this role is:115,523 - 150,244 USD per year(US National)
