As a Physical Therapist at Custom Home Health, you will:

• Deliver individualized care that empowers patients to achieve their health and independence goals.
• Collaborate with RNs and interdisciplinary teams to design and implement personalized care plans.
• Educate patients and their families on therapy techniques, exercises, and treatments to promote lasting well-being.
• Coordinate and administer treatments that enhance physical function and overall health.

• Licensed Physical Therapist: Must hold a valid Physical Therapy license.
• At Least 1 year of Home Health Experience with OASIS knowledge
