Bilingual QMHP – Full-Time (English/Spanish) in Dallas, Texas at Innovations Community Mental Health Center

Position Summary

The Bilingual QMHP-Dual provides psychosocial rehabilitation, skills training, and service coordination to adults with serious mental illness (SMI), as well as children and adolescents with serious emotional disturbance (SED). Services may include, but are not limited to, assessment, development of measurable treatment goals and objectives, referral, linkage, advocacy, monitoring, crisis intervention, skills training, psychosocial rehabilitation, and continuity of care.

The overall goal of this position is to maximize each individual’s potential level of functioning, reduce hospitalization, promote stability and independence, and aid in the successful integration or reintegration of individuals into the community.

Bilingual English/Spanish is required. Candidates must be able to read, write, and speak both English and Spanish proficiently and must be able to effectively provide services and communicate with clients and families in both languages.

ESSENTIAL DUTIES AND RESPONSIBILITIES: (Other duties may be assigned.)

• Provide overall service coordination, referral, linkage, and advocacy to individuals with varying behavioral health and community support needs. Perform duties in collaboration with other members of the interdisciplinary treatment team.
• Complete a psychosocial assessment on each new admission as assigned.
• Formulate an individualized recovery plan based on assessment findings for new admissions.
• Develop measurable objectives and goals in collaboration with the individual.
• Actively assist individuals in obtaining, reviewing, and modifying treatment goals as needed.
• Document attempts to involve the individual, service providers, caregivers, and other appropriate members of the treatment team in the service planning process unless opposed by the individual.
• Review recovery plans every three months for children and adults or as clinically indicated.
• Implement recovery plans for assigned individuals in accordance with authorized services and work toward objective and measurable outcomes established with the client.
• Actively engage all individuals assigned to the caseload and document services and engagement efforts as required.
• Participate in interdisciplinary treatment team staffing. Present psychosocial assessment findings, encourage individuals to participate in staffing when appropriate, update the treatment team regarding progress or lack of progress toward established goals, and provide relevant information that may affect treatment.
• Ensure appropriate authorization is obtained and maintained for clinical and rehabilitative services.
• Follow up with assigned individuals who miss scheduled appointments. Document attempts to contact the individual and report repeated unsuccessful contact attempts to the treatment team.
• Follow up within 24–48 hours with assigned individuals who access emergency services and report status and disposition to the treatment team.
• Meet with assigned individuals within three days of hospital discharge or assignment in the community, as required.
• Provide crisis intervention and support as appropriate. This may include home visits, assisting with mental health crisis procedures, and working with law enforcement, emergency personnel, and other community partners as needed to address a behavioral health crisis.
• Facilitate inpatient admission when clinically indicated and requested by the treatment team.
• Provide individual, family, and group psychosocial rehabilitation and skills training services. Evaluate progress, solicit feedback from individuals and families, and consult with colleagues and clinical leadership when appropriate.
• Identify and assist individuals in obtaining benefits, entitlements, and community resources through referral, linkage, advocacy, and coordination as needed.
• Provide culturally responsive services and effectively communicate with English- and Spanish-speaking clients, families, caregivers, and community partners.

Responsibilities

• Adhere to all applicable federal and state laws and Innovations Community Mental Health Center’s Policies and Procedures, including safeguarding confidential healthcare information and complying with the Health Insurance Portability and Accountability Act (HIPAA).
• Report suspected incidents of child abuse or neglect in accordance with applicable Texas mandatory reporting requirements.
• Report suspected incidents of abuse, neglect, or exploitation of vulnerable adults in accordance with applicable reporting requirements.
• Establish and maintain professional relationships and communication with clients, families, caregivers, caseworkers, probation officers, community providers, and staff as applicable.
• Communicate effectively with clients and families in both English and Spanish.
• Comply with all initial, annual, and position-specific training requirements.
• Attend staff, treatment team, and agency meetings as required.
• Complete other duties as directed by the Program Manager or designated supervisor.

Clients:

• Enter demographic data, contacts, services provided, and other required information for newly assigned clients into the designated Electronic Medical Record (EMR) within required timeframes.
• Facilitate the development of individualized service and recovery plans.
• Work with adults, children, families, and treatment teams to develop Crisis Plans designed to promote the safety and stability of the client, family, and community.
• Provide approved skills training and psychosocial rehabilitation interventions to clients and families to address identified needs, functional impairments, behaviors, and treatment goals.
• Coordinate and facilitate ongoing adult, child, family, treatment team, and other appropriate team meetings.
• Maintain an assigned caseload of clients and families and provide authorized services in accordance with client needs, treatment plans, program requirements, and agency expectations.
