Job Description

Manager, Compliance and Quality- Hospice

Visiting Nurse Health System (Visiting Nurse) is hiring for a dynamic, high EQ Manager, Compliance and Quality for our Hospice Division. We seek a The Manager of Compliance and Quality provides a “hands on” approach to compliance and administrative leadership to hospice healthcare operations with direct reports of the QAPI Coordinator, Educator. The Compliance Director will develop, lead, and manage all compliance services as well as proven success in implementing continuous quality improvement initiatives and ensuring best practices of compliance and clinical operations role.. The CQM will also be responsible for spearheading program accreditation and leading the agency to clinical excellence. This position interacts with all levels of the organization, customers, and patients and / or caregivers. The CQM will be individual must have high standards and be able to hold others accountable for their responsibilities. The Manager of Compliance and Quality will serve as the agencies Compliance Officer and QAPI coordinator.

In this position you will perform...

Coordination of Services:

  • Conduct comprehensive psychosocial assessments of hospice patients and families.
  • Provide counseling, emotional support, crisis intervention, and bereavement services.
  • Develop and implement individualized care plans to address social, emotional, financial, and environmental needs.
  • Connect patients and families with community resources, financial assistance, and government benefit programs.
  • Advocate for patients and families while serving as a liaison between the agency and community resources.
  • Collaborate with the interdisciplinary team to coordinate patient care and participate in care planning meetings.
  • Educate patients, families, and staff on hospice services, coping strategies, boundaries, and available support resources.
  • Maintain accurate, timely, and compliant documentation, assessments, and required regulatory paperwork.
  • Participate in the agency's on-call rotation and respond to patient and family needs as required.
  • Deliver compassionate, culturally sensitive, patient- and family-centered care while adhering to agency policies and regulatory standards.
  • Develops appropriate care plans in consultation with the client, client’s family, and service providers.
  • Documents all care management activity and service-related information.
  • Ensures that documentation is consistent with the format required by depart cognitive standards (i.e., progress notes reflect care plans.)
  • Maintains confidential case records on all clients.
  • Demonstrates the ability to follow through in a thorough and timely manner on tasks assigned by management team and requests made by patients/families, referral sources, and community.
  • Documents appropriate follow up on client needs whether related to Hospice services or other community resource needs.

Do you have a....

  • Graduate of an undergraduate degree in a clinical field. A Master’s Degree in Nursing, Healthcare Administration, Business Administration, or related fields is preferred or possess equivalent in skill and experience.

  • Must have RN License

  • Five years of experience in clinical experience and at least five years supervisory or management in a hospital, hospice or similar setting.
  • Hospice care and familiarity with post-acute transitions is a plus. Preferred candidates will have prior experience demonstrating the effective clinical and financial management of healthcare operations.
  • Must possess a thorough knowledge of laws and regulations applicable to healthcare operations, including Medicare Conditions of Participation. In addition, all candidates must be able to show an understanding of finance, and human resource management.
  • Demonstrates knowledge of theories, principles, practices, regulatory requirements and techniques of management and marketing home care.
  • Reliable transportation including a valid driver's license and proof of vehicle insurance
