Job Description

Summary of Position

The Patient Follow-up Coordinator focuses entirely on executing administrative follow-up to ensure our patients successfully get their appointments scheduled. This position requires focus on operational tracking, clear communication, and persistent outreach. It is ideal for a proactive, solutions-oriented individual who enjoys phone-based coordination and administrative task management.

Key Responsibilities

  1. You will act as the champion for the patient after a referral is sent, tracking submitted orders to ensure they actually turn into scheduled appointments.

    1. Track & Verify: Proactively contact external specialist facilities via phone and portal to check the status of pending medical referrals.

    2. Play Detective: If an order hits a roadblock—such as a specialist no longer accepting a patient’s insurance or closing their books to new patients—you will investigate, find an alternative using our resource guides, or contact the patient directly to update them.

    3. Drive Patient Outcomes: Ensure appointments are secured, preventing healthcare delays for our community.

    4. Adapt Quickly: Flex and cross-train with other coordinators on the team to balance out daily workloads.

Qualifications / Experience:

  1. Required:

    1. An excellent, friendly, and persistent phone presence. You aren't afraid to call a busy clinic to get an answer.

    2. You don't just see a problem; you actively look for a workaround or solution.

  2. Preferred:

    1. Prior experience in customer service, call centers, administrative roles, or medical reception is a major plus.

    2. No Heavy Clinical Background Required: If you are organized, motivated, and great with people, we will train you on the rest!

Skills & Competencies

  1. Strong analytical, organizational, and problem-solving skills.

  2. Strong written and verbal communication skills.

  3. Ability to manage multiple priorities and meet deadlines in a remote work environment.

  4. High level of accuracy and attention to detail.

Home Office:

All employees must have a HIPAA compliant workplace that is free of any distractions. The workplace must be in a room with a locked door to prevent accidental PHI disclosures. The home office must have high-speed internet and a CCS approved computer with two monitors.

Physical Demand:

Includes but is not limited to vision, hearing, repetitive motion, typing, sedentary, and extended viewing of a work environment computer screen. Reasonable accommodation may be made, with advanced notice, to enable individuals with disabilities to perform the essential functions and expectations of the position without compromising patient care.
