Employment Type: Part-time, non-exempt
Work Location: Revitalize Physical Therapy – Yorkville, Illinois (on-site outpatient clinic)2. Pay Range and Benefits
Compensation:
Anticipated pay range: $55.00 – $60.00 per hour, depending on experience and qualifications.
Sign-on Bonus: $2,000
New hires are eligible to receive 50% of the sign-on bonus after completing the 90-day introductory period and the remaining balance after 6 months of continuous employment. Receipt of the full bonus is contingent upon completing 12 months of employment. If employment ends before 12 months of continuous employment, the full amount must be reimbursed.

Patient care / clinical responsibilities:
- Evaluate patients and develop individualized treatment plans based on clinical findings and patient goals.
- Provide evidence-based physical therapy interventions for orthopedic and musculoskeletal conditions.
- Deliver hands-on care and therapeutic exercise instruction in an outpatient setting.
- Monitor patient progress and update treatment plans as appropriate.
- Educate and motivate patients throughout their recovery journey.
- Collaborate with other clinical team members to support continuity of care.
- Maintain a safe, clean, and professional treatment environment.
- Complete accurate, timely documentation in accordance with clinic procedures and payer requirements.
- Communicate effectively with patients, providers, and staff regarding treatment plans, scheduling needs, and follow-up care.
- Participate in staff meetings, training, and other clinic initiatives as needed.
- Follow clinic policies and procedures, including confidentiality and safety protocols.
- Support a positive patient experience and a collaborative work environment.
- Perform other related duties as assigned that are consistent with this role.
Physical ability to:
- Stand and walk for extended periods during a typical shift.
- Assist patients with therapeutic activities, transfers, gait, and positioning as appropriate.
- Lift, move, or position equipment and supplies weighing up to approximately 50 pounds, with or without reasonable accommodation.
- Frequently bend, reach, kneel, and move throughout treatment areas.
- Use a computer, telephone, and other office equipment for extended periods.
Education and credentials:
- Degree in Physical Therapy required; Master’s degree preferred.
- Current Physical Therapy License in the state of Illinois required.
- Prior outpatient orthopedic or musculoskeletal experience preferred.
- New graduates are welcome to apply; mentorship available.
- Strong interpersonal and communication skills.

- Ability to work collaboratively in a team-oriented clinical environment.
