Position & Responsibilities:

  • Complies with ethical and professional standards of conduct as set forth by the American Physical Therapy Association.
  • Practices and maintains patient safety and dignity by abiding by Federal, State, Local and Optimal policy & procedures as pertains to patient protected health information and HIPAA (Health Information Portability and Accountability Act).
  • Assists the Physician in evaluating client/patients by applying diagnostic and prognostic muscle, nerve, joint and functional ability tests and participates in the Physician’s Plan of Care.
  • Develops, documents, implements and re-evaluates a specific Plan of Care noting all treatment modalities and frequencies.
  • Treats patients to relieve pain and/or develop or restore function to maintain maximum performance.
  • Makes arrangements for out-patient services which cannot be provided in the home
  • Reports to the Physician the client/patient’s reaction to treatment and/or any changes in the client/patient’s condition.
  • Records clinical/progress notes in the patient’s clinical record weekly. Instructs client/patients and their families in the use of prosthetic, orthotic and assistive devices (cane, walkers, wheelchairs, etc.).
  • Instructs the family in the client/patient’s total physical therapy program and evaluates the home environment.
  • Instructs other involved agency personnel in the care of client/patients receiving physical therapy.
  • Attends appropriate case conferences and In-service presentations.
  • Plans for patient’s discharge, prepares discharge summaries and instructions
  • Participates in staff and agency development activities.
  • Performs related and other duties as required.
  • Reports to the Director of Nursing and designated Nursing Supervisor or to the Rehabilitation Manager.
  • Complies with agency Policies and Procedures as well as Employee Handbook detailing Human Resources policies.
  • Other duties / responsibilities that support the agency’s core values of Commitment, Advocacy, Respect, and Excellence.
  • Completes emergency preparedness training as identified as appropriate for this employee level.
  • Participates in exercises and drills for emergency preparedness, as required.
  • Other duties and tasks as assigned by the Optimal Home Care Emergency Command Center communication tree in the event of a local, regional or national emergency and/or disaster.

Requirements:
  • Holds an active license and can practice in the state of Colorado
  • Preferrable at least one year of experience as a Physical Therapist
  • Possesses adequate and effective written and verbal communication skills.
  • Possess adequate and effective interpersonal skills.
