CASE MANAGER in Hobe Sound, Florida at Project Lift Inc

Title: Case Manager

Status: Exempt

Reports to: Site Director

SUMMARY OF RESPONSIBILITIES:

Implements Project LIFT Programs by delivering and documenting all activities. Work is performed under the general direction of the Site Director.

SPECIFIC DUTIES AND RESPONSIBILITIES:

Provides and performs program delivery functions including the following;

• Delivery of the overall program activities in accordance with the mission and goals of the organization.
• Assists in the development of a program evaluation framework to assess the strengths of the program and identify areas for improvement.
• Ensures the program operates within Project LIFT’s policies and procedures.
• Assist in the collection and maintenance of records according to the confidentiality/privacy policy of the organization.
• Coordinates the delivery of services to increase effectiveness and efficiency.
• Performing needs assessments to determine each client’s level of physical and mental functioning, needs for support and services, and areas to target for improvement.

1. Collaborate with the clinical team to assess and address each client’s individual needs.
2. Engage and coordinate with community-based resources to support and resolve client needs.

• Complete program intake and client/family interview processes
• Assisting with workforce development curriculum as needed.
• Assist the Site Director with special projects.
• Other duties assigned by the Program Director and/or Chief Executive Officer.

• Ability to establish and maintain effective working relationships with colleagues in community organizations, staff, and parents.
• Effective written communication, organizational, and computer skills. Proficiency in word processing, e-mail, and internet usage.

QUALIFICATIONS:

Experience working with teens preferred.

PHYSICAL ACTIVITY/WORKING CONDITIONS:

While performing the duties of this job, the employee is regularly required to sit, talk and hear. The employee is frequently required to use hands to finger, handle, or feel and reach with hands and arms. The employee is also required to sit at a desk and use a computer. The employee is required to stand and walk. The noise level in the work environment varies. Specific vision abilities required by this job include close vision and ability to adjust focus. In addition, the employee must able to lift up to minimum of 25lbs, able to work in non-air conditioned environment and to stay on feet for extended periods of time.
