Home Health Aide (HHA) in Springfield, Massachusetts at Colony Care at Home INC

Make a difference in the lives of our clients! Colony Care at Home is seeking compassionate and dependable Home Health Aides to provide hands-on care and support to clients in their homes.

Responsibilities include:

• Personal care including bathing, dressing, toileting, and feeding
• Assistance with transfers, ambulation, and mobility
• Hoyer lift and full-transfer assistance as needed
• Meal preparation and light housekeeping
• Medication reminders and errands/appointments as outlined in the care plan
• Monitoring and reporting changes in client condition
• Completing daily documentation and maintaining client confidentiality

• Current 75-hour HHA certificate
• Valid driver’s license
• Must be 18 or older
• Reliable and compassionate with the ability to follow a client’s care plan
• Previous experience preferred, but not required

Please note: This is a hands-on position that may involve full transfers of clients weighing up to 200 lbs. and use of a Hoyer lift.
