The Clinical Nurse Manager serves as a liaison between participants, families, care providers, and community resources to ensure participants’ needs are met holistically. This position is primarily focused on health care management, health and wellness education, and ongoing evaluation of services for older adults with Alzheimer’s Disease and Related Dementias (ADRD) and their caregivers.

RESPONSIBILITIES
  1. Care Planning for the Participants:
    1. Conduct nursing assessments and monitor participants’ health status using input from participants, caregivers, providers, and the interdisciplinary team.
    2. Evaluate participants’ progress toward health goals and adjust plans accordingly.
    3. Monitor adherence to care plans across disciplines and ensure continuity of care.

  1. Responsibilities to Participants
    1. Initiate the nursing assessment and nursing portion of the person-centered care plan
    2. Oversee the delivery of participant services to ensure compliance with Center policies and Oversight Agency regulations
    3. Serve as a clinical resource and support for managing complex participant needs
    4. Provide direct support for participants who are experiencing emotional or behavioral dysregulation
    5. Provide medication oversight and ensure treatments are administered safely and according to physician orders, delegating where appropriate
    6. Maintain accurate and complete participant records and documentation
    7. Design and oversee health-related group programming that supports functional and cognitive well-being
    8. Be present on the floor during lunch to monitor dietary needs and participant safety during meals

  1. Responsibilities to the Staff
    1. Orient, supervise and train health aides and nursing students to their duties, clinical policies
    2. Ensure all staff are familiar with the procedures and requirements of the Washington Home Center
    3. Plan in-service and clinical staff meetings
    4. Participate in multi-disciplinary care plan meetings
    5. Complete PPD on new staff as needed and maintain a PPD roster
    6. Communicate with staff regarding the care of participants as events or changes occur
    7. Supervise LPN and/or Med Tech, when these positions are needed and filled

  1. Responsibilities to the Program
    1. Data and Quality Improvement
  1. Track and report on key health metrics and trends; participate in quality improvement initiatives and audits to improve clinical outcomes
    1. Manager internal and external incident reporting in compliance with organizational policies and regulatory requirements
    2. Manage Infection Control and Prevention within the Center
    3. Attend required trainings
    4. Develop/update Policies and Procedures as needed and at least annually
    5. Provide bimonthly therapeutic programs on health issues for the participants
    6. Manage the Center nursing budget
    7. Order Center nursing / medical supplies monthly
    8. Ensure compliance with Medicaid, Veterans Administration and DACL requirements; assist with on-site annual reviews
    9. Plan and conduct health and wellness presentations to staff and community as needed

  1. Family and Caregiver Support
    1. Provide support and education to families individually as needed
    2. Maintain contact with participant’s Primary Care Provider regarding changes in participant’s condition, medication adjustments or the need for additional services
    3. Maintain ongoing regular communication with all caregivers regarding participant’s health status.

  1. Emergency Response and Crisis Management
    1. Respond promptly and effectively to medical and behavioral emergencies, including performing first aid or CPR as needed
    2. Initiate appropriate emergency protocols, including taking vitals, calling EMS, notifying families, and documenting incidents
    3. Train and guide staff on emergency preparedness procedures and individual emergency care plans

  1. Other Responsibilities
    1. Network with other day centers and referral resources
    2. Other duties as assigned

  1. Qualifications:
    1. District of Columbia licensed Registered Nurse, required
    2. At least two years’ experience working with older adults
    3. Experience/familiarity with Adult Day Health Program, community nursing practice, or geriatric care, preferred
    4. Strong physical assessment skills
    5. Computer Literacy and basic knowledge of Microsoft Office, including Outlook, Word, and PowerPoint
    6. Experience working as a member of a multi-disciplinary team, preferred
    7. Able to lift 20 pounds
    8. Documentation of initial and annual negative PPD test or chest x-ray, required
    9. CPR and First Aid certification, required
