About This Position

Certified Nursing Assistant

POSITION OVERVIEW (non-exempt):

Assists patients with activities of daily living, provides basic nursing care, and assists in the maintenance of a safe and clean environment.

QUALIFICATIONS:

  • C.N.A. certification required
  • Current BLS

ACCOUNTABILITY:

Reports to Registered Nurse, Charge Nurse, or House Supervisor

DIRECT REPORTS:

None

POSITION-SPECIFIC REQUIREMENTS:

  • Assists in patient admission, transfer, transports, and discharge procedures.
    • Admissions – turns down bed, places gown at bedside, fills water pitcher if applicable, and assists with height and weight.
    • Discharges – gathers all patient belongings, transports patient to discharge vehicle, gives discharge slip to admission personnel, and reports discharge time to ward clerk upon return to the unit.
    • Transfers – gathers all patient belongings, assists with the transfer of the patient to the ambulance cart if applicable, transports patient to the discharge vehicle, gives discharge slip to admission personnel, and reports discharge time to ward clerk upon return to unit.
  • Provides general nursing care to assigned group of patients.
    • Participates in hourly rounding on assigned patients and addresses the 5 “P’s” with each rounding
    • Assists assigned patients to change position at least every 2 hours
    • Assists assigned patients with bathing including general range of motion who are on bedrest and unable to reposition themselves
    • General range of motion every evening to patients who are on bedrest and unable to reposition themselves.
    • Offer or assist with toileting every 2 hours on assigned patients.
    • Provides or assists with oral hygiene according to hospital policy on assigned patients.
    • Changes linens daily and maintains clean linens throughout the shift
    • Provides hair care as needed with bath
    • Provides nail care as needed with bath
    • Maintains room in clean neat order keeping visitor chair free of linens, etc
    • Assists appropriate patients to the chair for meals
    • Assists assigned patients to prepare for quiet time by addressing need for toilet, giving back rub, closing curtains, dimming lights, closing room door, etc.
    • Assists in handling and caring for patient personal property (dentures, glasses, contacts, hearing aids, prosthetic devices, and clothing) throughout the patient’s stay.
    • Clean clothes should be hung up or folded and placed in closet, dirty clothes should be placed in a black plastic bag and placed in closet for family to take home,
    • Soiled clothes that can’t be taken home immediately should be laundered and returned to the patient’s room.
  • Accurately measures and records intake, output, height, and weight – obtains and records identified weekly and daily weights prior to physician rounds
  • Assists with meals
    • Ensures NPO signs are posted appropriately
    • Ensures I/O sheets are in rooms
    • Prepares patients for meals
    • Passes and picks up trays
    • Records fluid intake and diet intake percentage on I/O sheet
    • Assists patients on room service to order meals as needed
    • Assists patients with meals as needed
    • Uses appropriate swallowing techniques with patients at risk for aspiration
    • Offers assigned patients some type of fluid intake every 2 hours and records that intake on the I/O sheet.
    • Passes nourishments at 100, 1400, and 2200
    • Records fluid intake and percentage of snack consumed on I/O sheet
    • Passes fresh water at 0700, 1500, and 2200
    • Orders kitchen supplies daily, maintains cleanliness of refrigerators, monitors for outdated food items.
    • Checks and records the temp of patient’s refrigerator daily
  • Participates in nursing plan of care, as directed.
    • Listens to report, takes note of diets, activities, DNR status, and other pertinent patient information
    • Maintains patient safety by using gaits belts for all patient transfers
    • Implements the use of egg crate mattresses on appropriate patients
    • Implements the use of heel protectors and heel elevation on appropriate patients.
    • Assess for heel elevation off bed when rounding
    • Implements the use of personal alarms on patients at risk for falling
    • Participates in preventative care of the patients by assisting them to take 10 deep breaths and cough and do 10 ankle pumps every 2 hours.
    • Maintains coffee, fruit, and cookies in nourishment center
    • Wears Vocera pager to facilitate prompt response to patient needs
    • Uses identified scripts consistently (i.e., “Is there anything else I can do for you, I have time?”), etc.
  • Attends all required meetings and in-services.
  • Assists with restocking and maintaining supplies for the unit. Participates with checking for outdated supplies on the unit.
  • 11-7 CNA will be trained to assist in ER and with registration of ER patients

WORK ENVIRONMENT:

  • Works in a well-illuminated, climate-controlled environment
  • May come in contact with hazardous chemical or treatment modalities
  • Possibility exists of exposure to communicable diseases
  • Involvement in inpatient care may result in unavoidable work-related illness or injury
  • Work environment is comfortable, with minimal exposure to physical hazards
  • Moderate to loud noise environment when machines/equipment in use
  • Involvement in checking in/moving of supplies may result in unavoidable work-related injury
  • Must be constantly alert for patient/family emergency situations
