Position Overview We are seeking an experienced Home Health Occupational Therapist who is passionate about patient-centered care. This role offers autonomy, flexibility, and collaboration with a strong interdisciplinary team to deliver high-quality therapy in patients’ homes.

Responsibilities Perform comprehensive occupational therapy evaluations

Develop and implement individualized treatment plans

Provide skilled OT interventions to improve daily functioning

Educate patients and caregivers on exercises, safety, and prevention

Monitor progress and adjust care plans as needed

Document all care accurately and timely in the EMR

Collaborate with physicians and care teams

Ensure compliance with Medicare, HIPAA, and regulatory standards

Supervise Certified Occupational Therapy Assistants (COTAs)

Qualifications Graduate of an accredited OT program

Current Virginia OT license required

Minimum 3 years of clinical experience (home health preferred)

Strong assessment, documentation, and care-planning skills

EMR experience preferred (Kinnser/WellSky or Homecare Homebase a plus)

Reliable transportation & valid driver’s license
