Team Lead - Case Management (RN or LMSW/LGPC) - Grace Medical Center at LifeBridge Health – Baltimore, Maryland

• Oversees the daily activities ensuring department standards are met, and provides real-time mentoring, advisement, and education to CM staff following CM processes
• Work with the CM Manager on the day-to-day activities of the department, including but not limited to assisting in creating staff schedules and ensuring adequate coverage.
• Quality and productivity monitoring of staff work and documentation in EMR and EMR tools.
• Guide staff toward desired outcomes, setting high-performance standards and delivering quality services.
• Communicates professionally, clearly, and assertively,

• Ability to work independently with little to no supervision.
• Strong organizational skills and ability to proactively prioritize needs and effectively manage resources with positive outcomes.

REQUIREMENTS:

• Minimum 1 year of experience; 3+ years of experience highly preferred
• Masters Degree in Social Work/Counseling
• LMSW/LGCP license in MD required; LCPC/LCSW-C preferred
• Case Management Certification preferred
• Will consider RNs that have dual training with MSW
