CHMC-Emergency Department - Psychosocial Assessor – Per Diem- 12-hour shift- Night Shift– Req 9456
Pay Range $26.40-$34.31*+
We are seeking a compassionate and detail-oriented Psychosocial Assessor to join our care coordination team. In this role, you will identify high-risk patients, conduct thorough assessments, and develop comprehensive admission and discharge plans that balance hospital goals with the unique needs of patients and their families.
Key Responsibilities

Assessment & Planning: Use high-risk screening criteria to evaluate patients and coordinate personalized care and discharge strategies.

Clinical Support: Provide crisis intervention, trauma-informed care, bereavement counseling, and supportive counseling tailored to cultural and gender-specific needs.

Advocacy: Serve as a bridge between medical staff and families, helping the team understand the social and emotional factors impacting a patient’s recovery.

Regulatory Compliance: Maintain strict adherence to state and federal regulations (NYS DOH, OMH, DNV) and fulfill all reporting mandates for abuse and neglect.

Safety & Intervention: Implement patient security procedures, including participating in crisis interventions and physically restraining combative residents when necessary.
Qualifications & Requirements

Education: Bachelor’s degree in Psychology (preferred).

Certifications: Current BLS and PMCS (Preventing and Managing Crisis Situations) certifications.

Compliance: Commitment to patient confidentiality (HIPAA), quality assurance, and maintaining a safe, clean environment.

Technical Skills: Proficiency in basic computer applications and accurate medical documentation.
