About This Position
About the Role:
The MDS/RN plays a critical role in ensuring the delivery of high-quality patient care through comprehensive assessment and care planning. This position is responsible for conducting thorough Minimum Data Set (MDS) assessments to evaluate residents' health status, functional capabilities, and care needs. The MDS/RN collaborates closely with interdisciplinary teams to develop individualized care plans that promote optimal health outcomes and regulatory compliance. This role requires meticulous documentation and adherence to federal and state guidelines to support reimbursement processes and quality improvement initiatives. Ultimately, the MDS/RN serves as a key liaison between clinical staff, residents, and families to foster a supportive and effective care environment.
Position: MDS Specialist (RN)
Location: Hybrid (2 Remote days and 3 travel days)
Responsibilities:
- Coordinate the Resident Assessment Instrument (RAI) process to ensure accurate and timely completion of resident assessments in accordance with Medicare, Medicaid, OBRA and other payor program requirements
- Ensure accuracy of RAI to reflect optimal reimbursement for services provided
- Ensure appropriate documentation to report and support services provided and assessment accuracy
Flexible schedule! 2 remote days and 3 days will float at the direction of the Director of Clinical Reimbursement
Requirements:
Current RN licensure
- Previous MDS 3.0 experience required (minimum 2 years)

Analytical skills are essential to interpret assessment results and identify changes in resident conditions that require intervention. Additionally, organizational skills help manage multiple assessments and care plans simultaneously, ensuring timely completion and quality outcomes.
