Job Responsibilities Perform in-home nursing assessments and skilled visits

Complete OASIS start of care, recertification, and discharge assessments

Develop and update individualized care plans

Provide skilled nursing care including wound care, medication management, IV therapy, and catheter care

Educate patients and caregivers

Coordinate care with interdisciplinary team members

Maintain accurate and timely documentation in the EHR

Follow infection control and safety standards

Qualifications Active Virginia RN license (LPN experience may be considered)

2–3 years of nursing experience (home health preferred)

Experience with wound care and nursing diagnoses preferred

Strong communication and clinical skills

Valid driver’s license and reliable transportation
