Summary: Provides patient care using the nursing processes of assessment, planning, implementation, and
evaluation of the delivery of health care services. All team members of Southeast Georgia Health System will

and national patient safety standards.

Reports To: Clinical Team Coordinator, Department Support Coordinator, Manager, Physician Practices

Essential Responsibilities: This description of job responsibilities is intended to reflect the major responsibilities and duties of the job, but
is not intended to describe minor duties or other responsibilities as may be assigned from time to time.
1. Effectively manages the care of assigned patients using knowledge, clinical skills, judgment and decision making skills for the specific area.
2. Delivers patient care utilizing guidelines of care and appropriate resources, including personnel, services, supplies and equipment.
3. Triages and assesses urgency of patient conditions.
4. Reviews, evaluates and takes appropriate action regarding results of diagnostic tests. Administers medications and IVs according to Health System policy. Assists with suture placement in treatment of lacerations.
5. Assist with patient flow by registering patients and assisting with other patient care duties.
6. Assists other healthcare professionals in performing exams, diagnostic procedures and treatments, and minor medical procedures. Intervenes in cardiac emergencies, possibly requiring CPR. Communicates provider advice/instructions.
7. Assists the clinical team in performing the nursing assessment to identify and prioritize physical, psychosocial and cultural needs in order to develop a plan of care.
8. Evaluates and reassesses nursing interventions based on interdisciplinary plan of care and makes revisions as appropriate.
9. Assists with the development and implementation of a patient/family education plan. Documents assessments, interventions, patient/family responses, and medication reconciliation and test results in the medical record.

Obtains prior authorizations as needed for further testing, procedures, medications, etc.

Minimum Qualifications:
Education Graduate of a Licensed Practical Nursing program.
Experience None required, but preferred.
Licensure Current licensure to practice as a LPN in the state of Georgia.
Knowledge/Skills/Abilities Nursing skills as defined in the law governing the practice of nursing in Georgia. American Heart Association BLS Certification required within first week of orientation and maintained throughout employment. Uses good judgment. Exhibits positive interpersonal skills.
